眶上外侧入路显微夹闭术对破裂前交通动脉瘤患者Hunt-Hess分级及预后优良率的影响

Effects of micro-clamping of lateral supraorbital approach on Hunt-Hess classification and prognostic and excellent rate of patients with pre-rupture communicating aneurysms

目的探讨眶上外侧入路显微夹闭术对破裂前交通动脉瘤患者Hunt-Hess分级及预后优良率的影响。方法回顾性分析2017年12月至2019年12月本院收治的98例破裂前交通动脉瘤患者的临床资料,按照手术入路方式分为对照组(翼点入路,n=45)与观察组(眶上外侧入路,n=53)。比较两组手术相关指标、预后优良率及并发症发生率。结果观察组切口长度、手术时间短于对照组,术中出血量、骨瓣大小均小于对照组(P<0.05)。观察组预后优良率高于对照组,并发症发生率低于对照组,但差异均无统计学意义。结论眶上外侧入路显微夹闭术在Ⅰ~Ⅲ级破裂前交通动脉瘤患者中应用效果良好,可有效改善预后,降低并发症发生率。

Objective To investigate the effect of micro-clamping of supraorbital approach on Hunt-Hess grading and prognosis of patients with pre-rupture communicating aneurysms.Methods The clinical data of 98 patients with pre-rupture communication aneurysms admitted in our hospital from December 2017 to December 2019 were retrospective analyzed.They were divided into control group(wing-point approach,n=45)and observation group(superior orbital approach,n=53)according to the surgical approach.The operation-related indexes,prognostic excellent rate,and complication rate between the two groups were compared.Results The incision length,operation time of the observation group were shorter than those of the control group,the intraoperative blood loss,bone flap size were less than the control group(P<0.05).The prognosis rate of the observation group was higher than that of the control group,and the incidence of complications was slightly lower than that of the control group,but there was no significant difference between the two groups.Conclusion The superior orbital lateral approach micro-clamping is effective in patients with gradeⅠ-Ⅲpre-rupture communicating aneurysms,which can effectively improve the prognosis and reduce the incidence of complications.
