婴幼儿医用粘胶相关性皮肤损伤危险因素的Meta分析

Meta Analysis of Risk Factors for Medical Adhesive-related Skin Injury in Infants

目的通过Meta分析明确婴幼儿住院期间发生医用粘胶相关性皮肤损伤(MARSI)的危险因素。方法计算机检索中国知网、万方、维普、中国生物医学文献数据库、Cochrane Library、PubMed、Web of Science、EMbase数据库关于婴幼儿医用粘胶相关性皮肤损伤危险因素的文献,采用RevMan5.3软件进行Meta分析。结果共纳入7篇文献,共17项危险因素。Meta分析结果显示,胎龄[OR=3.05,95%CI(1.87,4.98),P<0.00001]、蓝光治疗[OR=3.84,95%CI(3.40,4.35),P<0.00001]、留置胃管[OR=3.30,95%CI(1.28,8.48),P=0.01]、皮肤干燥[OR=5.29,95%CI(2.67,10.46),P<0.00001]、皮肤水肿[OR=4.11,95%CI(2.14,7.91),P<0.00001]是婴幼儿发生MARSI的危险因素。结论胎龄、蓝光治疗、留置胃管、皮肤干燥、皮肤水肿是婴幼儿发生MARSI的危险因素,护理过程中应加强监测,对可改变因素进行早期识别与干预,预防MARSI的发生。

Objective To identify the risk factors of medical adhesive-related skin injury(MARSI)in infants during hospitalization through Meta-analysis.Methods Searched CNKI,Wanfang,VIP,CBM,Cochrane Library,PubMed,Web of Science and EMbase databases for literatures on risk factors of medical adhesive-associated skin injury in infants and young children.Meta-analysis was performed using RevMan 5.3 software.Results A total of seven literatures were included,with a total of 17 risk factors.Meta-analysis results showed that the gestational age[OR=3.05,95%CI(1.87,4.98),P<0.00001],blue light treatment[OR=3.84,95%CI(3.40,4.35),P<0.00001],indwelling gastric tube[OR=3.30,95%CI(1.28,8.48),P=0.01],dry skin[OR=5.29,95%CI(2.67,10.46),P<0.00001],and skin edema[OR=4.11,95%CI(2.14,7.91),P<0.00001]were the risk factors for MARSI in infants.Conclusion The gestational age,blue light therapy,indwelling gastric tube,dry skin,and skin edema are the risk factors for MARSI in infants and young children.The monitoring should be strengthened in the nursing process,and the changeable factors should be identified and intervened early to prevent the occurrence of MARSI.
