摘要
目的探讨血嗜酸性粒细胞对肺结节良恶性鉴别的诊断价值。方法选择2017年1月-2019年12月在我院行胸部CT提示为肺结节,并经穿刺活检明确病理诊断的患者72例为研究对象;依据病理诊断分为恶性组43例,非恶性组29例,收集两组患者的临床资料并比较,包括年龄、血白细胞、血小板、血中性粒细胞、血淋巴细胞、血单核细胞、血白蛋白、血嗜酸性粒细胞、血嗜碱性粒细胞、血NLR、血总胆红素、血CEA、血CYFRA211、NSE、SCC-Ag,对恶性肺结节的危险因素进行单因素和多因素Logistic回归分析。结果两组血小板、血嗜酸性粒细胞、血CEA、血CYFRA211、血NSE比较,差异有统计学意义(P<0.05);两组年龄、血白细胞、中性粒细胞、淋巴细胞、单核细胞、血清白蛋白、血NLR、血总胆红素、SCC-Ag比较,差异无统计学意义(P>0.05);多因素Logistic回归分析表明,血嗜酸性粒细胞是恶性肺结节的独立危险因素。结论血嗜酸性粒细胞表达水平升高是诊断恶性肺结节的独立危险因素,对判断肺部结节的良恶性有重要的价值。
Objective To investigate the value of blood eosinophils in the diagnosis of benign and malignant pulmonary nodules.Methods From January 2017 to December 2019,72 patients with pulmonary nodules diagnosed by chest CT and confirmed by biopsy were selected as subjects.According to the pathological diagnosis,the patients were divided into malignant group(43 cases)and non-malignant group(29 cases).The clinical data of the two groups were collected and compared,including age,white blood cells,platelets,blood neutrophils,blood lymphocytes,blood monocytes,serum albumin,blood eosinophils,blood basophils,blood NLR,blood total bilirubin,blood CEA,blood CYFRA211,NSE and SCC-Ag.The risk factors of malignant pulmonary nodules were analyzed by univariate and multivariate Logistic regression analysis.Results The differences in platelet,blood eosinophils,serum CEA,serum CYFRA211 and serum NSE between the two groups were statistically significant(P<0.05).There was no significant difference in age,white blood cells,neutrophils,lymphocytes,monocytes,serum albumin,blood NLR,total bilirubin and SCC-Ag between the two groups(P>0.05).Multivariate Logistic regression analysis showed that blood eosinophils were independent risk factors for malignant pulmonary nodules.Conclusion Elevated blood eosinophil expression is an independent risk factor for the diagnosis of malignant pulmonary nodules,which has important value in judging benign and malignant pulmonary nodules.
