圆利针伞形针刺对寒凝湿滞型慢性肩周炎患者肩关节功能的影响

目的分析寒凝湿滞型慢性肩周炎患者应用圆利针伞形针刺对肩关节疼痛、活动及功能的影响。方法选取开封市中心医院康复医学科2018年7月至2020年10月收治的87例寒凝湿滞型慢性肩周炎患者作为研究对象,采用随机数字表进行分组,对照组43例给予口服双氯芬酸钠缓释片配合康复功能训练,观察组44例联合圆利针伞形针刺,对比两组患者临床疗效及各项指标。结果治疗后,观察组现时疼痛强度(PPI)、疼痛分级指数(PRI)、视觉模拟评分法(VAS)评分改善显著优于对照组(P<0.05);观察组Constant-Murley肩关节功能评分(CMS)、肩关节活动功能(Melle)评分水平均明显低于对照组,肩关节日常生活能力(ADL)评分高于对照组(P<0.05);观察组患者总有效率(97.73%)高于对照组(79.07%),P<0.05;对照组不良反应总发生率(2.33%)与观察组总发生率(2.27%)比较,差异无统计学意义(P>0.05)。结论圆利针伞形针刺联合双氯芬酸钠配合康复训练治疗寒凝湿滞型肩周炎患者,能够有效减轻患者疼痛,增加肩关节活动度,提升临床治疗效果。

Objective To analyze the effect of umbrella acupuncture with round sharp needles on shoulder pain,activity and function in patients with chronic scapulohumeral periarthritis of cold coagulation and dampness stagnation type.Me thods 87 patients with chronic scapulohumeral periarthritis of cold coagulation and dampness stagnation type treated in the Department of Rehabilitation Medicine of Kaifeng Central Hospital from July 2018 to October 2020 were selected as the research objects.They were divided into groups by random number table.43 cases in the control group were given oral Diclofenac Sodium sustained-release tablets combined with rehabilitation function training,and44 cases in the observation group were combined with umbrella acupuncture with round sharp needles.The clinical efficacy and indexes of the two groups were compared.Re s ults After treatment,the improvement of present pain intensity(PPI),pain rating index(PRI)and visual analogue scale(VAS)in the observation group was significantly better than that in the control group(P<0.05);The scores of Constant-Murley shoulder function(CMS)and shoulder activity function(Melle)in the observation group were significantly lower than those in the control group,and the scores of shoulder activity of daily living(ADL)were higher than those in the control group(P<0.05);The total effective rate of the observation group(97.73%)was higher than that of the control group(79.07%),P<0.05;The total incidence of adverse reactions in the control group(2.33%)was compared with that in the observation group(2.27%)(P>0.05).Conclus ion Umbrella acupuncture with round sharp needles and Diclofenac Sodium combined with rehabilitation training in the treatment of chronic scapulohumeral periarthritis of cold coagulation and dampness stagnation type can effectively reduce the pain,increase the range of motion of shoulder joint and improve the clinical treatment effect.
