摘要
目的探讨持续动脉灌注新辅助化疗对中晚期口腔癌患者客观缓解率、组织多肽特异性抗原(TPS)和糖类抗原125(CA125)水平的影响。方法选取2018年3月至2020年3月间金山区亭林医院收治的120例中晚期口腔肿瘤患者作为研究对象,采用信封法分组原则将以上患者随机分为观察组及对照组,每组60例。观察组患者采取持续动脉灌注新辅助化疗联合手术治疗,对照组患者采用手术治疗,两组患者术后均进行化疗。比较两组患者的治疗效果、围手术期指标、TPS、CA125水平、炎性反应水平以及不良反应。结果观察组患者的客观缓解率(45.0%)高于对照组(26.7%),差异有统计学意义(P <0.05),两组患者的手术时间、淋巴结清扫数目、出血量、术后引流量以及住院时间比较,差异无统计学意义(P> 0.05)。经过治疗后,两组患者的血清TPS和CA125水平均下降,且观察组患者的血清TPS水平低于对照组,差异均有统计学意义(均P <0.05)。两组患者术后感染、肝肾功能异常和骨髓抑制间比较,差异无统计学意义(P> 0.05)。结论中晚期口腔癌患者采取持续动脉灌注新辅助化疗,TPS、CA125水平显著下调,客观缓解率显著改善。
Objective To study the effects of neoadjuvant chemotherapy with continuous arterial infusion on the objective remission rate and the levels of tissue polypeptide specific antigen(TPS) and carbohydrate antigen 125(CA125) in patients with intermediate and advanced oral cancer. Methods A total of 120 patients with intermediate and advanced oral cancer were selected at Tinglin Hospital of Jinshan District between March 2018 and March 2020. They were randomly divided into an observation group and a control group with 60 patients in each group. The observation group received continuous arterial infusion for neoadjuvant chemotherapy combined with surgery and the control group underwent surgery. Both groups received chemotherapy after operation. The therapeutic effect,perioperative indicators,TPS,CA125 levels,inflammatory reaction levels and adverse reactions were compared between the two groups. Results The objective remission rate was significantly higher in the observation group(45. 0%) than in the control group(26. 7%)(P < 0. 05). There was no significant difference in operation time,number of lymph node dissected,blood loss,postoperative drainage and length of hospital stay between the two groups(P > 0. 05).After the treatment,the serum TPS and CA125 levels decreased significantly in the two groups,and the serum TPS levels were significantly lower in the observation group than in the control group(all P < 0. 05).There was no difference in postoperative infection,abnormal liver and kidney function and bone marrow suppression between the two groups(P > 0. 05). Conclusion The levels of TPS and CA125 were significantly decreased,and the objective remission rate was significantly improved in patients with intermediate and advanced oral cancer.
