摘要
目的探讨乳酸脱氢酶(LDH)对儿童腺病毒肺炎严重度的预测价值。方法回顾性分析深圳市儿童医院2017年10月至2019年12月488例住院诊断为腺病毒肺炎的患儿临床资料,根据病情轻重,分为重症组(125例)和轻症组(363例)。采用Mann-Whitney U检验比较两组患儿LDH的表达差异,应用ROC曲线对重症腺病毒肺炎的发生进行预测,确定特异性指标临界值。结果两组数据间比较表明,重症组LDH水平显著高于轻症组,差异具有统计学意义(P<0.05)。对LDH水平做ROC曲线分析,当LDH值为599U/L时,AUC(曲线下面积)为0.724,预测重症腺病毒肺炎的敏感性是57.6%,特异性是76.9%。结论LDH对于儿童腺病毒肺炎病情的严重程度有一定的预测价值,早期及时检测患儿血浆LDH水平对病情严重程度和预后的评估,以及早期积极干预治疗等都具有非常重要的临床意义。
Objective To explore the predictive value of lactate dehydrogenase(LDH)for the severity of adenovirus pneumonia in children.Methods A retrospective analysis of the clinical data of 488 hospitalized children diagnosed with adenovirus pneumonia from October 2017 to December 2019 in Shenzhen Children’s Hospital.According to the severity of the disease,it is divided into severe group(125 cases)and mild group(363 cases).The Mann-Whitney U test was used to compare the differences in the expression of LDH between the two groups of children,and the ROC curve was used to predict the occurrence of severe adenovirus pneumonia,and to determine the critical value of specific indicators.Results Pairwise comparison between the two groups of data showed that the LDH level of the severe group was significantly higher than that of the mild group,and the difference was statistically significant(P<0.05).Perform ROC curve analysis on the LDH level.When the LDH value is 599U/L,the AUC(area under the curve)is 0.724.The sensitivity to predict severe adenovirus pneumonia is 57.6%and the specificity is 76.9%.Conclusion LDH has a certain predictive value for the severity of childhood adenovirus pneumonia.Early and timely detection of children’s plasma LDH levels is of great clinical significance for the evaluation of the severity and prognosis of the disease,as well as early active intervention and treatment.
