腹腔镜手术治疗对子宫内膜癌患者的排尿功能、膀胱功能的影响

Effect of Laparoscopic Surgery on Micturition Function and Bladder Function in Patients with Endometrial Cancer

目的探究子宫内膜癌患者采用腹腔镜手术治疗的临床效果。方法选择88例子宫内膜癌患者,依据手术方法不同分为开腹组及腹腔镜组,各44例。开腹组行传统开腹子宫切除术,腹腔镜组于腹腔镜下行子宫切除术。对比两组围术期指标、手术前后膀胱功能、排尿功能指标。结果相比于开腹组,腹腔镜组肛门排气时间及住院时间均较短,术中出血量少,有统计学意义(P<0.05),两组手术时间、淋巴结清扫数目比较,无统计学意义(P>0.05);术后腹腔镜组最大尿流率、排尿量、平均尿流率、最大逼尿肌压力均高于开腹组,残余尿量少于开腹组,腹腔镜组膀胱壁厚度小于开腹组,有统计学意义(P<0.05)。结论子宫内膜癌患者给予腹腔镜手术治疗疗效确切,对术后排尿功能及膀胱功能影响较小。

Objective To explore the clinical effect of laparoscopic surgery in patients with endometrial cancer.Methods 88 patients with endometrial cancer were divided into laparotomy group and laparoscopy group,44 cases in each group.Traditional open hysterectomy was performed in the open group,and laparoscopic hysterectomy was performed in the laparoscopic group.The perioperative indexes,bladder function and micturition function indexes before and after operation were compared between the 2 groups.Results Compared with the open group,the anal exhaust time and hospital stay in the laparoscopic group were shorter,the amount of intraoperative bleeding was less,which was statistically significant(P<0.05).There was no statistically significant difference in the operation time and the number of lymph node dissection between the 2 groups(P>0.05);The maximum urinary flow rate,micturition volume,average urinary flow rate and maximum detrusor pressure in the laparoscopic group were higher than those in the open group,the residual urine volume was less than that in the open group,and the bladder wall thickness in the laparoscopic group was less than that in the open group(P<0.05).Conclusion Laparoscopic surgery is effective in the treatment of endometrial cancer,and has little effect on postoperative micturition function and bladder function.
