摘要
目的:观察机械振动排痰治疗老年社区获得性肺炎患者的效果。方法:选取96例老年社区获得性肺炎患者为研究对象,按照随机数字表法分为对照组和观察组各48例。对照组给予常规排痰治疗,观察组给予机械振动排痰治疗,比较两组临床疗效、症状消失时间、治疗前后肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、呼气流量峰值(PEF)]水平和动脉血气分析指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))]水平。结果:观察组治疗总有效率为95.82%,高于对照组的83.33%,差异有统计学意义(P<0.05);观察组喘息、气促、肺啰音和咳嗽等症状消失时间均短于对照组,差异有统计学意义(P<0.05);治疗后,观察组FVC、FEV_(1)、PEF等肺功能指标水平均高于对照组,差异有统计学意义(P<0.05);观察组PaO_(2)水平高于对照组,PaCO_(2)水平低于对照组,差异均有统计学意义(P<0.05)。结论:机械振动排痰治疗老年社区获得性肺炎患者可提高治疗总有效率和肺功能指标水平,改善动脉血气分析指标水平,效果优于单纯常规排痰治疗。
Objective:To observe effects of mechanical vibration expectoration on 96 elderly patients with community-acquired pneumonia.Methods:96 elderly patients with community-acquired pneumonia were selected as the research objects,and were divided into control group and observation group according to the random number table method,48 cases in each group.The control group was given routine expectoration therapy,while the observation group was given the mechanical vibration expectoration therapy.The clinical efficacy,the symptom disappearance time,the pulmonary function index levels[forced vital capacity(FVC),forced expiratory volume in 1 second(FEV_(1)),peak expiratory flow(PEF)]and the arterial blood gas analysis index levels[arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2))]before and after the treatment were compared between the two groups.Results:The total effective rate of the observation group was 95.82%,which was higher than 83.33%of the control group,and the difference was statistically significant(P<0.05).The disappearance time of symptoms such as wheezing,short breath,lung rales and cough in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of FVC,FEV_(1),PEF and other pulmonary function indexes in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Further,the level of PaO_(2) in the observation group was higher than that in the control group;the level of PaCO_(2) in the observation group was lower than that in the control group;and the differences were statistically significant(P<0.05).Conclusions:Mechanical vibration expectoration in the treatment of the elderly patients with community-acquired pneumonia can improve the total effective rate of treatment,the level of pulmonary function indexes,and the levels of arterial blood gas analysis indexes.Moreover,it is superior to single routine expectoration therapy.
