小青龙汤加味联合孟鲁司特钠治疗小儿咳嗽变异性哮喘患儿的效果

Effects of Xiaoqinglong decoction combined with Montelukast sodium in treatment of children with cough variant asthma

目的:观察小青龙汤加味联合孟鲁司特钠治疗小儿咳嗽变异性哮喘患儿的效果。方法:选取72例小儿咳嗽变异性哮喘患儿为研究对象,按照随机数字表法分为对照组和观察组各36例。对照组给予孟鲁司特钠治疗,观察组在对照组基础上联合小青龙汤加味治疗。比较两组治疗前后血清免疫球蛋白E(IgE)、细胞间黏附分子-1(ICAM-1)、转化生长因子-β_(1)( TGF-β_(1))和呼出气一氧化氮(FeNO)水平,用药第1、14天呼气峰流速(PEF)日间变异率,第1、2周PEF变异率及出院后4个月复发率。结果:治疗后,两组IgE、ICAM-1、TGF-β_(1)、FeNO水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);第14天,两组PEF日间变异率均明显低于第1天,且观察组低于对照组,差异有统计学意义(P<0.05);第2周,两组PEF周变异率均低于第1周,且观察组第1、2周PEF周变异率均低于对照组,差异有统计学意义(P<0.05);观察组复发率为2.78%,明显低于对照组的22.22%,差异有统计学意义(P<0.05)。结论:小青龙汤加味联合孟鲁司特钠治疗小儿咳嗽变异性哮喘患儿可降低TGF-β_(1)、FeNO、ICAM-1和IgE水平、PEF日变异率、PEF周变异率和复发率,优于单纯孟鲁司特钠治疗效果。

Objective:To observe effects of Xiaoqinglong decoction combined with Montelukast sodium in treatment of children with cough variant asthma.Methods:72 children with cough variant asthma were selected as the research objects,and were divided into control group and observation group according to the random number table method,36 cases in each group.The control group was treated with Montelukast sodium granules,while the observation group was treated with Xiaoqinglong decoction on the basis of that of the control group.The levels of serum immunoglobulin E(IgE),intercellular adhesion molecule-1(ICAM-1),transforming growth factor-β_(1)(TGF-β_(1))and exhaled nitric oxide(FeNO),the daytime variation rate 1 and 14 days of medication,the PEF variation rate at 1 and 2 weeks,and the recurrence rate 4 months after discharge were compared between the two groups before and after the treatment.Results:After the treatment,the levels of IgE,ICAM-1,TGF-β_(1) and FeNO in the two groups were lower than those before the treatment;those of the observation group were lower than those of the control group;and the differences were statistically significant(P<0.05).On the 14th day,the peak expiratory flow(PEF)daytime variation rates of in the two groups were significantly lower than that on the first day;that of the observation group was lower than that of the control group;and the differences were statistically significant(P<0.05).In the second week,the PEF weekly variation rates in both groups were lower than those in the first week;the PEF weekly variation rates in the observation group were lower than those in the control group in the first and second weeks;and the differences were statistically significant(P<0.05).Further,the recurrence rate of the observation group was 2.78%,which was significantly lower than the control group of 22.22%,and the difference was statistically significant(P<0.05).Conclusions:Xiaoqinglong decoction combined with Montelukast sodium in the treatment of the children with cough variant asthma can reduce the levels of TGF-β_(1),FeNO,ICAM-1 and IgE,the PEF daytime variation rate,the PEF weekly variation rate and the recurrence rate.Moreover,it is superior to single Montelukast sodium.
