宣肺补益汤联合GSH对血液透析合并COPD患者治疗效果的影响

Clinical efficacy of Xuanfeibuyi decoction combined with GSH for hemodialysis patients with COPD

目的分析宣肺补益汤联合还原型谷胱甘肽(GSH)治疗合并慢性阻塞性肺疾病(COPD)血液透析患者的临床价值。方法按照随机数字表法将2018年1月至2020年3月收治的62例合并COPD的血液透析患者分为观察组和对照组,每组31例。常规治疗基础上,对照组给予GSH治疗,观察组给予GSH+宣肺补益汤治疗,2组均治疗2周。对比2组治疗前后肾功能[胱抑素C(CysC)、尿素氮(BUN)、肌酐(Scr)]、肺功能[第1秒用力呼气量(FEV_(1))、用力肺活量(FVC)、1秒率(FEV_(1)/FVC)]及生活质量(WHOQOL-BREF)改善情况,对比2组治疗安全性。结果治疗后2组血清CysC、BUN、Scr水平较治疗前降低,且观察组低于对照组(P<0.05);治疗后2组FEV_(1)、FVC、FEV_(1)/FVC水平均较治疗前升高,且观察组高于对照组(P<0.05);治疗后2组WHOQOL-BREF各维度评分均较治疗前增加,且观察组高于对照组(P<0.05);观察组不良反应发生率为9.68%与对照组6.46%比较差异无统计学意义(P>0.05)。结论针对合并COPD的血液透析患者,实施宣肺补益汤联合GSH治疗方案,可取得显著效果,有利于改善患者肾功能及肺功能、提高生存质量,且安全性有保障。

Objective To investigate the clinical efficacy of Xuanfeibuyi decoction combined with reduced glutathione(GSH)in the treatment of hemodialysis patients with chronic obstructive pulmonary disease(COPD).Methods A total of 62 hemodialysis patients with COPD who were treated in our hospital from January 2018 to March 2020 were enrolled in the study,who were randomly divided into observation group and control group,with 31 cases in each group.On the basis of conventional treatment,the patients in control group were treated by GSH,however,the patients in and the observation group were treated by GSH+Xuanfeibuyi decoction,with a treatment course of 2 weeks for both groups.The renal function indexes[cystatin C(CysC),urea nitrogen(BUN),creatinine(SCR)],pulmonary function indexes(FEV_(1),FVC,FEV_(1)/FVC)]and quality of life(WHOQOL-BREF)were observed and compared between the two groups.Results After treatment,the serum levels of CysC,BUN and SCR in both groups were significantly decreased,moreover,which observation group were significantly lower than those in control group(P<0.05).After treatment,the levels of FEV_(1),FVC and FEV_(1)/FVC in both groups were significantly increased,moreover,which in observation group were significantly higher than those in control group(P<0.05).In addition the scores of WHOQOL-BREF in both groups were significantly increased,moreover,which in observation group were significantly higher than those in control group(P<0.05).In addition there was no significant difference in the incidence rate of adverse reactions between observation group and control group(9.68%vs 6.46%,P>0.05).Conclusion Xuanfeibuyi decoction combined with GSH can obtain significant curative effects for hemodialysis patients with COPD,which can improve the renal function and lung function,improve the life quality of patients,with good safety.
