宫腔镜联合腹腔镜手术治疗子宫内膜异位症并不孕症的临床疗效探析

Clinical efficacy of hysteroscopy combined with laparoscopic surgery in the treatment of endometriosis with infertility

目的探讨子宫内膜异位症并不孕症患者应用宫腔镜联合腹腔镜手术治疗的临床疗效。方法 118例子宫内膜异位症并不孕症患者,根据手术方式不同分为腹腔镜组与联合组,每组59例。腹腔镜组单纯应用腹腔镜手术治疗,联合组采用宫腔镜联合腹腔镜手术治疗。对比两组患者手术时间、术中出血量、术后住院时间及1、2年妊娠率。结果联合组患者的手术时间(75.01±11.45)min、术中出血量(60.02±4.12)ml、术后住院时间(7.56±1.03)d,与腹腔镜组的(74.34±11.03)min、(62.15±7.96)ml、(7.61±1.02)d对比,差异无统计学意义(P>0.05)。联合组患者术后1、2年妊娠率分别为52.5%、74.6%,均明显高于腹腔镜组的33.9%、49.2%,差异具有统计学意义(P<0.05)。结论宫腔镜联合腹腔镜手术治疗子宫内膜异位症并不孕症患者,具有明确的治疗效果,可有效提升妊娠率,值得临床推广应用。

Objective To discuss the clinical efficacy of hysteroscopy combined with laparoscopic surgery in the treatment of endometriosis with infertility.Methods A total of 118 patients with endometriosis and infertility were divided into laparoscopic group and combined group,according to different surgical methods with 59 cases in each group.The laparoscopic group was treated with laparoscopic surgery alone,and the combined group was treated with hysteroscopy and laparoscopic surgery.The operation time,intraoperative blood loss,postoperative hospital stay,and pregnancy rates at 1 and 2 years postoperatively were compared between the two groups.Results The operation time of the combined group was(75.01±11.45) min,the intraoperative blood loss was(60.02±4.12) ml,and the postoperative hospitalization time was(7.56±1.03) d,which were not statistically significant compared with(74.34±11.03) min,(62.15±7.96)ml and(7.61±1.02) d of the laparoscopic group(P>0.05).The pregnancy rates at 1 and 2 years postoperatively of the combined group were 52.5% and 74.6%,which were significantly higher than 33.9% and 49.2% of the laparoscopic group,and the difference was statistically significant(P<0.05).Conclusion Hysteroscopy in combination with laparoscopic surgery has definite therapeutic effect on patients with endometriosis and infertility,and it can effectively improve the pregnancy rate,which is worthy of clinical promotion and application.
