口腔颌面部囊肿实行开窗减压术治疗的临床疗效研究

Clinical efficacy of fenestration decompression in the treatment of oral and maxillofacial cysts

目的分析开窗减压术治疗口腔颌面部囊肿的临床疗效。方法 62例口腔颌面部囊肿患者,依据随机数字表法分为研究组与对照组,各31例。对照组应用囊肿刮除术治疗,研究组应用开窗减压术治疗。比较两组治疗效果、临床指标、术后囊腔情况及并发症发生情况。结果研究组治疗总有效率93.55%高于对照组的74.19%,差异有统计学意义(P<0.05)。研究组手术时间、住院时间短于对照组,术中出血量、术后视觉模拟评分法(VAS)评分均低于对照组,差异均有统计学意义(P<0.05)。研究组囊腔缩小率、囊腔缩小体积及术后囊腔密度分别为(83.21±8.22)%、(51.21±4.22)mm^(2)、(0.53±0.15)g/m^(2),均高于对照组的(62.24±7.64)%、(36.63±3.98)mm^(2)、(0.34±0.12)g/m^(2),差异均有统计学意义(P<0.05)。研究组并发症发生率6.45%低于对照组的25.81%,差异有统计学意义(P<0.05)。结论对口腔颌面部囊肿患者应用开窗减压术治疗可显著改善其病情,此术式与囊肿刮除术相比更具优势,建议将此术式在临床中推广应用。

Objective To analyze the clinical efficacy of fenestration decompression in the treatment of oral and maxillofacial cysts.Methods A total of 62 patients with oral and maxillofacial cysts were divided into research group and control group according to random numerical table,with 31 cases in each group.The control group was treated with curettage of cysts,and the research group was treated with fenestration decompression.The therapeutic effects,clinical indicators,postoperative cyst conditions and complications were compared between the two groups.Results The total effective rate of treatment 93.55%in the research group was higher than 74.19%in the control group,and the difference was statistically significant (P<0.05).The operation time and hospitalization time of the research group were shorter than those of the control group,and the intraoperative blood loss and postoperative visual analogue scale (VAS) scores were lower than those of the control group.All the differences were statistically significant (P<0.05).The reduction rate of the cyst cavity,the reduction volume of the cyst cavity,and the density of the postoperative cyst cavity in the research group were (83.21±8.22)%,(51.21±4.22) mm^(2),and (0.53±0.15) g/m^(2),which were higher than (62.24±7.64)%,(36.63±3.98) mm^(2),and (0.34±0.12) g/m^(2) in the control group,and the differences were statistically significant (P<0.05).The complication rate 6.45%of the research group was lower than 25.81%of the control group,and the difference was statistically significant (P<0.05).Conclusion The application of fenestration decompression in the treatment of oral and maxillofacial cysts can significantly improve the condition of the patients,and this operation has more advantages than curettage of cyst.It is suggested to be promoted and applied in clinical practice.
