酶联免疫吸附试验诊断梅毒螺旋体感染的临床价值

The clinical value of enzyme-linked immunosorbent assay in the diagnosis of Treponema pallidum infection

目的探究梅毒螺旋体(TP)感染中应用酶联免疫吸附试验(ELISA)诊断的临床价值。方法 150例疑似梅毒螺旋体感染患者,分别采用酶联免疫吸附试验及快速血浆反应素环状卡片试验(RPR)进行诊断,以梅毒螺旋体明胶凝集试验(TPPA)检测结果为金标准,分析梅毒螺旋体感染阳性和阴性患者的检测结果,并对比酶联免疫吸附试验及快速血浆反应素环状卡片试验诊断梅毒螺旋体感染的价值。结果 150例疑似梅毒螺旋体感染患者中,经梅毒螺旋体明胶凝集试验检测后阳性100例、阴性50例。阳性患者经酶联免疫吸附试验检测的S/CO值高于阴性患者,差异具有统计学意义(P<0.05);阳性患者经快速血浆反应素环状卡片试验检测的滴度高于阴性患者。以梅毒螺旋体明胶凝集试验检测结果为金标准,酶联免疫吸附试验诊断的灵敏度98.00%高于快速血浆反应素环状卡片试验的80.00%,漏诊率2.00%低于快速血浆反应素环状卡片试验的20.00%,差异有统计学意义(P<0.05);酶联免疫吸附试验和快速血浆反应素环状卡片试验诊断的特异度、误诊率及阳性预测值对比,差异无统计学意义(P>0.05)。结论酶联免疫吸附试验的应用对梅毒螺旋体感染患者的检测灵敏度较高,具有一定的临床应用及采纳价值。

Objective To investigate the clinical value of enzyme-linked immunosorbent assay(ELISA in the diagnosis of Treponema pallidum(TP) infection.Methods For 150 patients suspected Treponema pallidum infection,enzyme linked immunosorbent assay(ELISA) and rapid plasma reagin ring card test(RPR) were used for diagnosis,and Treponema pallidum gelatin agglutination test(TPPA) test results were considered as the gold standard.The test results of positive and negative patients with Treponema pallidum infection were analyzed,and the values of enzyme-linked immunosorbent assay and rapid plasma reagin ring card test in the diagnosis of Treponema pallidum infection were compared.Results Among 150 patients suspected Treponema pallidum infected,100 cases were positive and 50 cases were negative in the Treponema pallidum gelatin agglutination test.The S/CO value of positive patients detected with enzyme-linked immunosorbent assay was significantly higher than those of negative patients,and the difference was statistically significant(P<0.05).The results of titer of rapid plasma reagin ring card test of positive patients was higher than those of negative patients.The results of Treponema pallidum gelatin agglutination test were considered as the gold standard,the sensitivity of the enzymelinked immunosorbent assay was 98.00%,which was higher than 80.00% of the rapid plasma reagin ring card test,and the misdiagnosis rate of the enzyme-linked immunosorbent assay was 2.00%,which was lower than 20.00% of the rapid plasma reagin ring card test;and the differences were statistically significant(P<0.05).The differences in the specificity,misdiagnosis rate and positive predictive value between the enzyme-linked immunosorbent assay and the rapid plasma reagin ring card test were not statistically significant(P>0.05).Conclusion The enzymelinked immunosorbent assay has high sensitivity in the detection of patients with Treponema pallidum infection.Thus,it has certain value in clinical applications.
