超分子水杨酸换肤治疗中重度寻常痤疮的疗效及不良反应

Study for the efficacy and adverse reactions of supramolecular salicylic acid peel in treatment of moderate-to-severe acne vulgaris

目的评估超分子水杨酸换肤联合米诺环素治疗中重度寻常痤疮的疗效,探究水杨酸换肤不良反应的发生特点,为临床提供实用性参考。方法纳入176例中重度寻常痤疮患者。对照组服用米诺环素(100 mg/日,分2次),疗程6周;研究组在米诺环素基础上,应用30%超分子水杨酸换肤治疗2次(第0、2周)。比较两组GAGS评分变化,并分析超分子水杨酸换肤的不良反应特点。结果两组治疗后GAGS评分均显著低于治疗前(P<0.01),且研究组降幅更显著(P<0.01)。治疗前两组间GAGS评分无统计学差异,治疗后研究组显著低于对照组(P<0.01)。换肤治疗即刻,所有患者均出现面部皮肤轻度红肿,持续1~7 d,平均(2.9±0.8)d;治疗结束后1~3 d,平均(1.9±0.5)d,所有患者均出现面部皮肤干燥/脱屑,持续1~7 d,平均(3.4±1.1)d;仅1例发生色素沉着;无皮肤接触致敏者。结论超分子水杨酸换肤不良反应轻微可控,其联合米诺环素口服治疗中重度寻常痤疮疗效确切。

Objective To evaluate the clinical efficacy of supramolecular salicylic acid peel and oral minocycline combination therapy in moderate-to-severe acne vulgaris as well as to explore the adverse reactions of supramolecular salicylic acid peel,providing a reference for the clinical treatment.Methods Totally 176 patients with moderate-to-severe acne vulgaris were included in the study.The control group was given oral minocycline 50 mg twice a day for 6 weeks.In addition to oral medication,the study group was also given two sessions of supramolecular salicylic acid 30%peel at week 0 and week 2,respectively.The GAGS scores were compared and analyzed.The adverse reactions were also monitored.Results GAGS scores after the treatment in two groups were both significantly lower than those before the treatment(P<0.01),and the decrease was more significant in the study group.Before the treatment,there was no significant difference in GAGS scores between the two groups,while after the treatment,GAGS scores of the study group were significantly lower than those of the control group(P<0.01).After the peeling therapy,all patients presented with an immediate skin redness and mild swelling which lasted for(1~7)days(mean,2.90.8 days).One to three days(mean,1.90.5 days)after peeling,patients presented with facial dryness/desquamation,lasting for(1~7)days(mean,3.41.1 days).Only one patient suffered post-inflammatory hyperpigmentation.No contact dermatitis occurred.Conclusion The combination therapy of supramolecular salicylic acid peeling and oral minocycline is effective in moderate-to-severe acnes with mild adverse reactions.
