经阴道彩色多普勒超声联合弹性成像技术在子宫恶性肿瘤早期诊断中的应用价值

Application Value of Transvaginal Color Doppler Ultrasound and Ultrasound Elastography Technology in Early Diagnosis of Uterine Malignant Tumor

目的探讨经阴道彩色多普勒超声(TVCDU)联合弹性成像(UE)技术在子宫恶性肿瘤早期诊断中的应用价值。方法选取2018年5月至2020年5月在商丘市睢阳区中心医院就诊的138例疑似子宫恶性肿瘤患者作为研究对象。所有患者均接受TVCDU、UE检查,以手术病理检查结果为“金标准”,比较TVCDU、UE单独及联合检查的诊断结果、诊断效能及对不同类型子宫恶性肿瘤的检出率,并比较早、晚期子宫恶性肿瘤患者的血液显示率、子宫内膜厚度、阻力指数等指标。结果手术病理检查结果显示,138例患者中有124例恶性病变,TVCDU检查显示有104例恶性病变,UE检查显示有107例恶性病变,经TVCDU及UE联合检查共发现122例恶性病变。与TVCDU、UE单独检查相比,联合检查灵敏度(94.35%)、准确率(91.30%)及对子宫颈癌、子宫内膜癌的检出率较高,漏诊率(5.65%)较低,差异有统计学意义(P<0.05)。与ⅠA期患者相比,ⅠB期患者血液显示率、子宫内膜厚度、阻力指数较高,差异有统计学意义(P<0.05)。结论TVCDU及UE联合检查的诊断准确率高于单独检查,联合检查可有效提高诊断效能及对不同类型子宫恶性肿瘤的检出率,对临床早期筛查诊断、制定治疗方案具有重要意义。

Objective To explore the application value of transvaginal color Doppler ultrasound(TVCDU)combined with ultrasound elastography(UE)in the early diagnosis of uterine malignant tumors.Methods A total of 138 patients with suspected uterine malignant tumors in the Suiyang District Central Hospital of Shangqiu City from May 2018 to May 2020 were selected as the study subjects.All patients underwent TVCDU and UE examination.The results of surgical pathological examination were regarded as the“gold standard”.The diagnostic results,diagnostic efficacy and detection rate of different types of uterine malignant tumors were compared of TVCDU and UE individual and joint inspection.The blood display rate,endometrial thickness and resistance index of patients with early and late uterine malignant tumors were compared.Results The results of surgical and pathological examination confirmed that 124 of 138 patients had malignant lesions,and there were 104 cases of malignant lesions by TVCDU,107 cases by UE,and 122 cases by TVCDU and UE combined examination.Compared with TVCDU and UE alone examinations,the sensitivity(94.35%),accuracy(91.30%)and the detection rate of cervical and endometrial cancer were higher,and the missed diagnosis rate(5.65%)was lower,and the difference was statistically significant(P<0.05).Compared with stageⅠA patients,the blood display rate,endometrial thickness and resistance index of stageⅠB patients were higher,and the difference was statistically significant(P<0.05).Conclusion The diagnostic accuracy rate of TVCDU and UE combined examination is higher than that of single test.It can effectively improve the diagnostic efficiency and the detection rate of different types of uterine malignancies.It is of great significance for early clinical screening and diagnosis and formulating treatment plans.
