摘要
目的回顾性分析超声引导下射频消融对甲状腺微小乳头状癌(PTMC)体积的影响。方法收集2016年1月至2020年4月因PTMC到郑州大学第一附属医院超声介入科行射频消融的患者71例,于术后3个月(±14d)、6个月(±14d)、9个月(±14d)、12个月(±14d)随访观察术后患者结节体积大小,计算术后不同时期结节体积大小及体积吸收率。结果术后3个月(±14d)、6个月(±14d)、9个月(±14d)、12个月(±14d)体积分别为176.2(79.7,413.7)mm^(3)、56.0(22.9,133.1)mm^(3)、20.8(10.1,58.7)mm^(3)、6.4(2.7,15.9)mm^(3),平均体积吸收率分别为-243.5%、-65.7%、21.4%、64.5%。术后6个月(±14d)2例结节消失,3例缩小为条索样低回声;术后9个月(±14d)10例结节消失,4例仅可见钙化;术后12个月(±14d)14例结节消失,6例仅可见钙化。术后1a临床效果评价:结节消失(痊愈)14例;疗效显著(平均体积吸收率50%~99%)45例,其中5例仅可见钙化;有效(平均体积吸收率25%~49%)9例,其中1例仅可见钙化;仍大于原始结节3例,其中1例伴粗大钙化。结论射频消融治疗PTMC后结节体积较原始结节体积明显缩小,术后吸收效果好。
Objective To retrospectively analyze the effect of ultrasound-guided radiofrequency ablation on nodular volume of papillary thyroid microcarcinoma.Methods From January 2016 to April 2020,71 patients underwent radiofrequency ablation in the Department of intervention Ultrasound of the First Affiliated Hospital of Zhengzhou University.Postoperative nodular volume wasobservedat3months(±14d),6months(±14d),9months(±14d)and12months(±14d),andthenodular volume and volume absorption rate was calculated at different postoperati ve periods.Results The mean volume at 3 months(±14 d),6 months(±14d),9 months(±14d)and 12 months(±14 d)postoperatively were 176.2(79.7,413.7)mm^(3)、56.0(22.9,133.1)mm^(3)、20.8(10.1,58.7)mm^(3)、6.4(2.7,15.9)mm^(3) respectively.The volume absorption rate after operation were-243.5%,-65.7%,21.4%and 64.5%,respectively.At 6 months(±14 d)postoperatively,2 cases of nodule disappeared,3 cases were reduced to cable-like hypoecho.At9 months(±14 d)postoperatively,10 cases of nodule disappeared,and 4 cases only showed caleification.At 12 months(+14 d)postoperatively,14 cases of nodules disappeared,and 6 cases only showed caleification.Clinical effect evaluation 1 year after operation:14 cases of nodules disappeared(healed),the curative effect were significant(absorptivity 50%-99%)in 45 cases,of which only calcification were visible in 5 cases,and 9 cases were effective(absorptivity 25%-49%),of which only caleification was visible in 1 case.The nodules were still larger than the original nodules in 3 cases,including 1 case with coarse caleification.Conclusion The nodular volume of PTMC after radiofrequeney ablation was si gnificantly smaller than the original nodular volume,and the absorption effect is good after operation.
