不同手术方式治疗卵巢囊肿患者的效果比较

Comparison of the Effect of Different Surgical Methods in Patients with Ovarian Cyst

目的探究无入路平台经脐单切口腹腔镜术(TU-SILS)与多孔腹腔镜术(MPLS)治疗卵巢囊肿患者的临床效果。方法选取2018年1月至2020年6月开封市人民医院收治的96例卵巢囊肿患者作为研究对象,根据手术方法分为TU-SILS组(n=48)、MPLS组(n=48)。TU-SILS组接受无入路平台TU-SILS治疗,MPLS组接受MPLS治疗。对比两组围手术期指标、卵巢功能指标[促卵泡生成素(FSH)、促黄体生成素(LH)、雌二醇(E_(2))水平]、炎症因子水平[血清C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)]及切口满意度。结果TU-SILS组手术时间、术后排气时间、下床活动时间、住院时间均短于MPLS组,术中出血量低于MPLS组(P<0.05);两组术后FSH、LH水平较术前升高,E_(2)水平较术前降低(P<0.05),但两组术后FSH、LH、E_(2)水平比较,差异无统计学意义(P>0.05);术后1d,两组血清CRP、TNF-α水平均升高,观察组较对照组低(P<0.05);TU-SILS组切口总满意度[95.83%(46/48)]高于MPLS组[81.25%(39/48)](P<0.05)。结论对卵巢囊肿实施TU-SILS与MPLS治疗均不影响卵巢功能恢复,与MPLS相比,TU-SILS可减少术中出血量,促进术后恢复,减轻对炎症因子水平的影响,提高患者切口满意度。

Objective To explore the elinical efect of the treatment of non-access platform transumbilical single-incision laparoscopic surgery(TU-SILS)and multi-port laparoscopic surgery(MPLS)in patients with ovarian cyst.Methods Ninety-six patients with ovarian cyst admitted to Kaifeng People’s Hospital from January 2018 to June 2020 were selected as the research objects.According to surgical methods,they were divided into TU-SILS group(n=48)and MPIS group(n=48).The TU-SILS group received the no-access platform TU-SILS,and the MPIS group received MPIS.Perioperative indicators,ovarian function indicators[follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2))levels],inflammatory factor levels[serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)]and incision sat isfaction of the two groups were compared.Results The surgery time,postoperative exhaust time,time to get out of bed,and hospital stay in the TU-SILS group were shorter than those in the MPIS group,and intraoperative blood loss was lower than that in the MPIS group(P<0.05).The levels of FSH and LH were higher and the level of E,was lower than that before surgery(P<0.05),but there was no si gnificant difference in the levels of FSH,LH and E_(2) between the two groups after surgery(P>0.05).One day after surgery,the serum CRP and TNF-αlevels increased,and the observation group was lower than the control group(P<0.05).The total satisfaction of incision in the TU-SILS group[95.83%(46/48)]was higher than that of the MPIS group[81.25%(39/48)](P<0.05).Conclusion Both TU-SILS and MPLS treatment of ovarian cysts do not afect the recovery of ovarian function.Compared with MPLS,TU-SIIS can reduce intraoperative blood loss,promote postoperative recovery,and reduce the levels of inflammat ory factors and improve patient sat isfaction with incisions.
