摘要
目的分析血清单核细胞趋化蛋白-1(MCP-1)、白细胞介素-1β(IL-1β)与糖尿病肾病患者糖代谢的关系。方法选择2020年9月至2021年9月息县中心医院收治的120例糖尿病患者,参照《中国成人糖尿病肾脏病临床诊断的专家共识》中标准,将患者分为早期糖尿病肾病组、临床期糖尿病肾病组、单纯糖尿病组,所有患者入院时均接受血清MCP-1、IL-1β水平检测及糖代谢指标检查[空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)],统计3组患者基线资料,分析血清MCP-1、IL-1β水平与糖尿病肾病患者糖代谢的关系。结果120例糖尿病患者中,单纯糖尿病88例(73.33%),确诊糖尿病肾病32例(26.67%),其中早期糖尿病肾病21例,临床期糖尿病肾病11例;临床期糖尿病肾病、早期糖尿病肾病组入院时血清MCP-1、IL-1β水平高于单纯糖尿病组,且临床期糖尿病肾病组高于早期糖尿病肾病组,差异有统计学意义(P<0.05);临床期糖尿病肾病组、早期糖尿病肾病组FPG、HbA1c、FINS、HOMA-IR水平高于单纯糖尿病组,且临床期糖尿病肾病组高于早期糖尿病肾病组,差异有统计学意义(P<0.05);相关性检验结果显示,糖尿病肾病患者血清MCP-1、IL-1β水平与糖代谢指标(FPG、HbA1c、FINS、HOMA-IR)呈正相关(r>0,P<0.05)。结论血清MCP-1、IL-1β水平与糖尿病肾病患者糖代谢存在一定联系。
Objective To analyxe the relationship between serum monocyte chemoattractant protein-1(MCP-1),interleukin-1β(IL-1β)and glucose metabolism in patients with diabetic nephropathy.Methods A total of 120 patients with diabetes melitus treated in Xi County Central Hospital from September 2020 to 2021 were selected.Referring to the standard of Expert Consensus on Clinical Diagnosis of Adult Diabetic Nephropathy in China,the pat ients were divided into early diabetic nephropathy group,elinical diabetic nephropathy group and simple diabetes group.All patients received detection of serum MCP-1,IL-1βlevels and examination of glucose metabolism indexes[fasting plasma glucose(FPG),hemoglobin Ale(HbA1c),fasting insulin(FINS),insulin resistance index(HOMA-IR)],and the baseline data of patients in the three groups were counted.The relationship between serum MCP-1 and IL-1βlevels and glucose metabolism in patients with diabetic nephropathy was analyzed.Results Among the 120 patients with diabetes,88 cases(73.33%)were simple diabetes,and 32 cases(26.67%)were diagnosed as diabetic nephropathy.Among them,there were 21 cases of early diabetic nephropathy and 11 cases of clinical diabetic nephropathy.The levels of serum MCP-1 and L-1βin clinical diabetic nephropathy group and early diabetic nephropathy group were higher than those of simple diabetes group,and the clinical diabetic nephropathy group was higher than that of early diabetic nephropathy group(P<0.05).The levels of FPG,HbAlc,FINS and HOMA-IR in clinical diabetie nephropathy group and early diabetic nephropathy group were higher than those in simple diabetes group,and the clinical diabetic nephropathy group was higher than that in early diabetic nephropathy group(P<0.05).Correlation test results showed that,the levels of serum MCP-1 and IL-1βwere positively correlated with glycometabolism indexes(FPG,HbAlc,FINS,HOMA-IR)in patients with diabetic nephropathy(r>0,P<0.05).Conclusion Serum MCP-1 and IL-1βlevels are associated with glucose metabolism in patients with diabetic nephropathy.
