肛周脓肿内口封闭+负压封闭引流术联合中药坐浴法治疗高位肛周脓肿的效果观察

Observation on the effect of sealing the inner orifice of perianal absces s+negative pressure sealing drainage combined with Traditional Chinese medicine sitting bath in the treatment of high perianal abscess

目的:研究用肛周脓肿内口封闭+负压封闭引流术联合中药坐浴法治疗高位肛周脓肿的临床疗效。方法:将2019年1月至2021年1月期间重庆市江津区中医院收治的150例高位肛周脓肿患者作为研究对象。采用随机数表法将其分为对照组(n=75)和观察组(n=75)。为对照组患者采用肛周脓肿内口封闭+负压封闭引流术进行治疗,在此基础上为观察组患者采用中药坐浴法进行治疗,然后比较两组患者治疗的总有效率、术后7 d视觉模拟评分法(VAS)的评分、术后创面红肿的情况、治疗后其创面腐肉脱落的时间及创面愈合的时间。结果:(1)观察组患者治疗的总有效率〔94.7%(71/75)〕高于对照组患者治疗的总有效率〔77.3%(58/75)〕,差异有统计学意义(P<0.05)。(2)观察组患者术后7 d的VAS评分〔(4.31±1.31)分〕低于对照组患者术后7 d的VAS评分〔(6.61±2.15)分〕,差异有统计学意义(P<0.05)。(3)治疗后,观察组患者创面腐肉脱落的时间、创面愈合的时间均短于对照组患者,差异有统计学意义(P<0.05)。(4)观察组患者术后创面红肿等级的分布情况优于对照组患者,相关数据差异有统计学意义(P<0.05)。结论:用肛周脓肿内口封闭+负压封闭引流术联合中药坐浴法治疗高位肛周脓肿的效果显著,可有效地改善患者创面疼痛、红肿的情况,促进其创面的愈合。

Objective:To study the clinical efficacy of perianal abscess internal mouth sealing+negative pressure sealing drainage combined with Traditional Chinese medicine sitting bath in the treatment of high perianal abscess.Methods:A total of 150 patients with high perianal abscess admitted to Jiangjin District Hospital of Traditional Chinese Medicine in Chongqing from January 2019 to January 2021 were selected as the subjects.They were divided into control group(n=75)and observation group(n=75)by random number table method.The control group was treated with perianal abscess internal mouth sealing+negative pressure sealing drainage,and the observation group was treated with traditional Chinese medicine sitting bath method on this basis.Then,the total effective rate,visual analogue scale(VAS)score at 7 d after surgery,postoperative wound redness and swelling,the time of wound decaying and wound healing after treatment were compared between the two groups.Results:(1)The total effective rate in observation group(94.7%(71/75))was higher than that in control group(77.3%(58/75)),and the difference was statistically significant(P<0.05).(2)VAS score of observation group 7 d after surgery was lower than that of control group 7 d after surgery((4.31±1.31)points(6.61±2.15)points),with statistical significance(P<0.05).(3)After treatment,the time of shedding of carrion and healing of wound in observation group was shorter than that in control group,with statistical significance(P<0.05).(4)The distribution of postoperative wound redness grade in the observation group was better than that in the control group,with statistically significant differences(P<0.05).Conclusion:The treatment of high perianal abscess with internal mouth sealing and negative pressure sealing drainage combined with traditional Chinese medicine sit-bath has a significant effect,which can effectively improve the situation of wound pa in,redness and swelling,and promote the wound healing.
