超声连续节段扫查法对小儿消化道梗阻病因的诊断价值

Diagnostic value of continuous segment ultrasound scan in children with alimentary tract obstruction

目的探讨小儿先天性上消化道梗阻中超声连续节段扫查法的临床诊断价值。方法选取2019年1月至2020年6月本院通过手术确诊为先天性上消化道梗阻患儿56例作为研究对象,均实施X线消化道造影与超声连续节段扫查法检查,比较两种检查方法对各种疾病的诊断率与诊断准确率。结果X线消化道造影与超声连续节段扫查法检查均可诊断先天性肥厚性幽门狭窄、肠旋转不良并中肠扭转、十二指肠狭窄、环状胰腺,超声连续节段扫查法诊断率明显高于X线消化道造影(P<0.05)。超声连续节段扫查法检查提示梗阻率为98.21%(55/56),高于X线消化道造影的82.14%(46/56)(χ^(2)=8.166,P=0.004);超声连续节段扫查法的诊断准确率为96.43%(54/56),高于X线消化道造影的71.43%(40/56),差异有统计学意义(χ^(2)=12.974,P=0.000)。结论小儿先天性上消化道梗阻中超声连续节段扫查法的临床诊断价值显著,可对多种疾病实施诊断,且超声连续节段扫查法的小儿先天性上消化道梗阻诊断准确率高于X线消化道造影。

Objective To investigate the clinical diagnostic value of continuous segment ultrasound scan in children with congenital upper gas-trointestinal obstruction.Methods From January 2016 to June 2019,56 cases confirmed by surgery for congenital upper gastrointestinal obstruction in children in our hospital were selected as the research subject,both the implementation of the digestive tract by X-ray imaging and continuous seg-ment ultrasound scan method,comparing two checking methods for the diagnosis of various diseases and diagnostic accuracy.Results X-ray gastro-intestinal tract and continuous segmental ultrasound examination can diagnose congenital hypertrophic pyloric stenosis,intestinal malrotation and midgut volvulus,duodenal stenosis,and circular pancreas,the diagnostic rate of ultrasound continuous segmental scanning is significantly higher than that of X-ray gastrointestinal angiography(P<0.05).Continuous segment ultrasound scan revealed that the obstruction rate was 98.21%(55/56),which was higher than 82.14%(46/56)of X-ray gastrointestinal contrast(χ^(2)=8.166,P=0.004);the diagnostic accuracy rate of continuous seg-mental ultrasound scanning was 96.43%(54/56),which was higher than the 71.43%(40/56)of X-ray gastrointestinal tract imaging,and the differ-ence was statistically significant(χ^(2)=12.974,P=0.000).Conclusion In children with congenital upper digestive tract obstruction,continuous seg-ment ultrasound scan has significant clinical diagnostic value,which can be used for the diagnosis of a variety of diseases,and the diagnostic accura-cy of continuous segmental ultrasound scan is higher than that of X-ray gastrointestinal angiography.
