硬膜外麻醉联合低剂量缩宫素对阴道试产产妇镇痛分娩的应用效果

Effect of epidural anesthesia combined with low dose oxytocin on analgesic delivery of vaginal trial delivery women

目的探讨硬膜外麻醉联合低剂量缩宫素对阴道试产产妇镇痛分娩的应用效果。方法选取2019年6月至2020年6月于本院进行阴道试产的60例产妇作为研究对象,按照是否要求无痛分娩分为观察组和对照组,每组30例。观察组实施硬膜外麻醉联合低剂量缩宫素镇痛分娩,对照组进行常规阴道试产,比较两组产妇中转剖宫产率、疼痛程度、总产程时间、产后出血量以及新生儿Apgar评分。结果观察组产妇剖宫产率为6.7%,低于对照组的33.3%,差异有统计学意义(P<0.05)。观察组产妇痛疼程度评分低于对照组,产后出血量少于对照组,总产程时间短于对照组,差异有统计学意义(P<0.05)。两组产妇分娩后新生儿Apgar评分比较差异无统计学意义。结论硬膜外麻醉联合低剂量缩宫素在阴道试产产妇镇痛分娩中的应用效果显著,有利于缓解产妇疼痛、缩短产程,提高自然分娩率,同时对新生儿无明显不良影响,值得临床实践应用。

Objective To explore the effect of epidural anesthesia combined with low dose oxytocin on analgesic delivery of vaginal trial deliv-ery women.Methods From June 2019 to June 2020,60 pregnant women who had vaginal trial delivery in our hospital were selected as the research subjects,they were divided into observation group and control group according to whether painless delivery is required,with 30 cases in each group.The observation group was given epidural anesthesia combined with low dose oxytocin analgesia,while the control group was given routine vaginal trial delivery.The conversion rate of cesarean section,pain degree,total labor process time,postpartum hemorrhage volume and neonatal Apgar score were compared between the two groups.Results The cesarean section rate of puerperae in the observation group was 6.7%,which was lower than 33.3%in the control group,and the difference was statistically significant(P<0.05).The maternal pain score in the observation group were lower than the control group,postpartum hemorrhage volume less than the control group and the total labor process time was shorter than the con-trol group,and the differences were statistically significant(P<0.05).There was no significant difference in Apgar score between the two groups.Conclusion Epidural anesthesia combined with low dose oxytocin in the application of vaginal trial labor analgesia delivery effect is significant,is conducive to alleviate maternal pain,shorten the labor process,improve the natural delivery rate,at the same time,it has no obvious adverse effect on newborns,which is worthy of clinical application.
