二维超声特殊切面联合三维超声自由解剖成像技术在胎儿唇腭裂中的应用 被引量:1

摘要 目的运用二维超声特殊切面联合三维自由解剖成像(Omniview)技术对胎儿唇腭裂进行诊断分析。方法回顾性分析产前超声检查的孕妇资料,利用二维超声常规切面及二维超声特殊切面共诊断唇腭裂或可疑唇腭裂胎儿60例,选取其中57例成功获得三维超声Omniview图像的病例作为研究对象,按孕龄将其分为3组(分别为<20周、20~25周、>25周),分别记录每组病例获得二维与三维图像情况以及诊断结果,最后对所有患者进行随访,将产后或引产后结果与产前超声检查进行对比,计算超声检查诊断符合率。结果超声检查诊断符合率在<20周时为75.%(12/16)、20~25周时为96.97%(32/33)、>25周75%(6/8),3组间比较差异有统计学意义(P<0.05);在单独二维超声与二维联合三维超声Omniview检查技术,2组间比较差异有统计学意义(P<0.05)。结论二维超声特殊切面联合三维超声Omniview检查技术能够直观地显示病变位置并明显提高胎儿唇腭裂的检出率,可帮助临床评估及孕妇咨询。 Objective To analyze the diagnosis of fetal cleft lip and palate by using the two-dimensional ultrasound special section combined with three-dimensional ultrasound omniview imaging technology.Methods The pregnant women for prenatal ultrasound examination were analyzed retrospectively.A total of 60 fetuses with cleft lip and palate or suspected cleft lip and palate were diagnosed by two-dimensional ultrasound conventional section and two-dimensional ultrasound special section.Among them,57 cases of three-dimensional ultrasound omniview imaging were received successfully and these cases were divided into three groups(including<20 weeks,20~25 weeks,>25 weeks)according to the gestational age.The two-dimensional and three-dimensional images and diagnostic results of each group were recorded and followed up.The results of postpartum or induced labor were compared with prenatal ultrasound,and the coincidence rate of ultrasound diagnosis was calculated.Results In the group of less than 20 weeks,the coincidence rate of ultrasound diagnosis was 75%(12/16).In the group of 20~25 weeks,it was 96.97%(32/33)and it was 75%(6/8)in the group of more than 20 weeks,there was significant difference(P<0.05),but there was significant difference between the group of single two-dimensional ultrasound and the group of two-dimensional combined with three-dimensional omniview(P<0.05).Conclusion Two-dimensional ultrasound special section combined with three-dimensional ultrasound omniview can directly display the location of lesions and significantly improve the detection rate of fetal cleft lip and palate,which can help clinical evaluation and pregnant women consultation.
