男性轻重度尼古丁依赖者动态低频振幅波动的磁共振研究

Magnetic Resonance Study of Dynamic Low-Frequency Amplitude Fluctuation in Light and Heavy Male Nicotine Dependents

目的探讨尼古丁依赖者静息态状况下特异的脑活动改变,以及重度尼古丁依赖者是否具有更特异性脑活动改变。方法对63例尼古丁依赖者(轻度组33例和重度组30例)和30例健康对照行静息态磁共振成像扫描并计算动态低频波动振幅(dALFF),对三组ALFF值行ANOVA分析。结果与健康对照组相比,轻度尼古丁依赖者在左侧额上/中回及右侧眶额中回dALFF变异性增加(t=2.91);重度尼古丁依赖者在左侧额上/中/下回、右侧眶额中回、右侧岛叶、右侧中央前/后回以及右侧额中回dALFF变异性增加(t=2.91)。与轻度尼古丁依赖者比较,重度尼古丁依赖者在右侧中央前回、右侧中央后回以及左侧中央前回dALFF变异性增加(t=2.91)。结论尼古丁依赖者前额叶的动态自发脑活动改变随着吸烟程度的进展稳定且持续存在,可能与吸烟行为有关。重度尼古丁依赖者中出现感知及运动相关皮层的动态自发脑活动的改变,可能与较严重的依赖程度有关。

Objective To investigate the specific changes of brain activity in resting state of nicotine dependent patients, and whether there are more specific changes of brain activity in severe nicotine dependent patients. Methods 63 nicotine dependent patients(33 cases in mild group and 30 cases in severe group) and 30 healthy controls were scanned by resting state magnetic resonance imaging and dALFF was calculated. ALFF values of the three groups were analyzed by ANOVA. Results Compared with healthy controls, dALFF variability increased in the left superior/middle frontal gyrus and the right orbitofrontal gyrus(t=2.91) in the light nicotine dependents;dALFF variability increased in left superior/middle/inferior frontal gyrus, right orbitofrontal gyrus, right insula, right anterior/posterior central gyrusand right medial frontal gyrus(t=2.91) in the heavy nicotine dependents.Compared with light nicotine dependence, heavy smokers were found increased dALFF variability in right precentral gyrus, right posterior central gyrus and left anterior central gyrus(t=2.91). Conclusion The dynamic spontaneous brain activity changes in the prefrontal lobe of nicotine dependent patients were stable and persistent with the progression of smoking degree, which may be related to smoking behavior. And the dynamic spontaneous brain activity changes in the perceptual and motor related cortex in severe nicotine dependent patients may be related to the more severe degree of dependence.
