耳鼻咽喉头颈外科医疗纠纷现状分析及防御对策研究

Analysis of medical disputes in otorhinolaryngology and head and neck surgery and research on defense countermeasures

目的通过分析耳鼻咽喉头颈外科诊疗特点以及医疗纠纷现状,提出针对耳鼻咽喉头颈外科医疗纠纷的防御对策及干预措施。方法对医务人员和患者医疗纠纷的认识等情况以及既往医疗纠纷病例进行调查分析,提出减少或避免耳鼻咽喉头颈外科医疗纠纷的防御对策,并运用于临床。以2016年全年医疗纠纷数量及患者对医疗满意度等指标为对照组,2017年运用干预措施后的相应指标为观察组,分析两组间的数据,观察医疗纠纷干预的效果。结果得到医方和患方影响耳鼻咽喉头颈外科医疗纠纷的基本情况;实施医疗纠纷干预措施后观察组比较对照组全年的医疗纠纷数量下降,患者对医疗满意度明显提高,差异有统计学意义(P<0.05)。结论耳鼻咽喉头颈外科医疗纠纷的预防及处理依赖多方面的因素,应制定相应的医疗防御对策,平衡多方利益,维护正常的诊疗秩序,促进医疗事业的健康发展和社会的稳定。

Objective To analyze the diagnosis and treatment characteristics of otorhinolaryngology head and neck surgery and the current situation of medical disputes,and to put forward defense countermeasures and intervention measures for medical disputes in otorhinolaryngology head and neck surgery.Methods The understanding of medical disputes between medical staff and patients and the previous cases of medical disputes were investigated and analyzed,and defensive countermeasures to reduce or avoid medical disputes in otorhinolaryngology head and neck surgery were put forward and applied clinically.Taking the number of medical disputes and patients'satisfaction with medical care in 2016 as the control group,and the corresponding indicators after intervention in 2017 as the observation group,the data between the two groups were analyzed to observe the effect of medical dispute intervention.Results The basic situation of medical disputes between doctors and patients affecting otorhinolaryngology head and neck surgery was obtained.After the implementation of medical dispute intervention measures,the number of medical disputes in the observation group decreased compared with that in the control group throughout the year,and the patients'satisfaction with medical treatment was significantly improved,with statistically significant difference(P<0.05).Conclusion The prevention and treatment of medical disputes in otorhinolaryngology head and neck surgery depend on various factors.Corresponding medical defense countermeasures should be formulated to balance the interests of various parties,maintain the normal diagnosis and treatment order,and promote the healthy development of medical undertakings and social stability.
