盐敏感性高血压收缩压负荷与肾素-血管紧张素-醛固酮系统、皮质醇相关性研究

Correlation among systolic blood pressure load,renin-angiotensin-aldosterone system and cortisol in patients with salt-sensitive hypertensive

目的探讨盐敏感性高血压患者24 h收缩压负荷(24h SBPL)与肾素-血管紧张素-醛固酮系统(RAAS)成分及血清皮质醇(COR)之间的关联。方法通过收纳2015年6月至2016年6月于中国中医科学院广安门医院心血管科符合盐敏感性高血压的住院患者,按照动态血压参数24h SBPL进行分组:收缩压负荷≥50%为高负荷组,<50%为低负荷组;测定患者RAAS系统中血清血管紧张素Ⅰ(AngⅠ)、血管紧张素Ⅱ(AngⅡ)、醛固酮(ALD)及HPA轴系统血清皮质醇(COR),比较两组之间上述成分有无差异。结果两组患者RASS系统中ALD水平高负荷组显著高于低负荷组,差异有统计学意义(P<0.05)。两组患者24h SBPL分别与ALD成正相关(r=0.310,P<0.001),与COR成正相关(r=0.556,P<0.001)。结论盐敏感高血压患者收缩压负荷可能与ALD及COR存在一定相关性。

Objective To discuss the correlation among 24h systolic blood pressure load(24hSBPL),renin-angiotensin-aldosterone system(RAAS)and cortisol(COR)in patients with salt-sensitive hypertensive(SSHT).Methods The patients with SSHT were chosen from Department of Cardiovascular Medicine in Guang`anmen Hospital of China Academy of Chinese Medical Sciences from June 2015 to June 2016.All patients were divided,according to 24h SBPL,into high SBPL group(SBPL≥50%)and low SBPL group(SBPL<50%).The levels of angiotensinⅠ(AngⅠ),angiotensinⅡ(AngⅡ)and aldosterone(ALD)in RAAS and COR level in hypothalamus-pituitary-adrenal axis(HPAA)were detected and compared between 2 groups.Results The level of ALD in RASS was significantly higher in high SBPL group than that in low SBPL group(P<0.05).24h SBPL was positively correlated to ALD(r=0.310,P<0.001),and positively correlated to COR(r=0.556,P<0.001)in 2 groups.Conclusion SBPL may be correlated to ALD and COR in patients with SSHT.
