针灸刺穴联合持续被动运动在膝关节置换术后康复中的应用

Application of Acupuncture Combined with Continuous Passive Motion in Postoperative Rehabilitation of Patients with Knee Arthroplasty

目的探讨针灸刺穴联合持续被动运动(CPM)在膝关节置换(TKA)术后康复中的应用效果。方法选择2019年9月—2020年9月医院收治的88例行TKA治疗患者,按随机数字表法分为两组,各44例。对照组采取COM治疗,观察组采取针灸刺穴联合CPM治疗,比较两组康复效果、治疗前及疼痛(视觉模拟评分发[VAS])及膝关节功能评分(膝关节评分[HSS])及膝关节恢复情况(膝关节活动度、膝关节周径)。结果治疗后,观察组康复有效率高于对照组(P<0.05);治疗后,两组VAS评分低于治疗前,HSS评分高于治疗前,观察组VAS评分低于对照组,HSS评分高于对照组(P<0.05);治疗后,两组膝关节活动度高于治疗前,膝关节周径低于治疗前,观察组膝关节活动度高于对照组,膝关节周径低于对照组(P<0.05)。结论针灸刺穴联合持续被动运动能够有效改善膝关节置换患者膝关节功能,降低术后疼痛,提升膝关节活动能力,提高临床康复效果,同时安全性较高。

Objective To investigate the effect of acupuncturecombined with continuous passive motion(CPM) in the postoperative rehabilitation of patients with knee arthroplasty(TKA).Methods A total of 88 patients treated with TKA in our hospital from September 2019 to September 2020 were prospectively selected.According to the random number table method,the patients were divided into two groups,44 cases in each group.The control group was intervened by COM,and the observation group was intervened by acupuncturecombined with CPM.The rehabilitation effect,pain[visual analogue scale(VAS)]and score of knee joint function[knee joint score(HSS)]and recovery of knee joint(knee joint range of motion,knee joint circumference) before and 15 days after intervention were compared between the two groups.Results After intervention,the effective rate of rehabilitation in the observation group was higher than that of the control group(P<0.05).After intervention,the VAS score of both groups was lower and the HSS score was higher than that before intervention.The VAS score of the observation group was lower and the HSS score was higher than that of the control group(P<0.05).After intervention,the range of motion of knee joint in both groups was higher and the circumference of knee joint was lower than those before intervention.The motion rangeof knee joint in the observation group was higher and the circumference of knee joint in the observation group was lower than those of the control group(P<0.05).Conclusion Acupuncturecombined with continuous passive motion can effectively improve the function of knee joint,reduce postoperative pain,improve the activity of knee joint motion and the clinical rehabilitation effect in patients with knee arthroplasty,and has high safety.
