膜迷路积水患者静态和动态背景下主观视觉垂直线、主观视觉水平线的表现

The characteristics of subjective visual vertical and subjective visual horizontal deviation angles of patients with endolymphatic hydrops under the static and dynamic backgrounds

摘要 目的分析膜迷路积水患者静态和动态背景下主观视觉垂直线(SVV)、主观视觉水平线(SVH)偏斜角度的特点。方法选取44例临床确诊膜迷路积水的患者作为观察组和30例正常人作为对照组。所有人均进行静态和动态背景下SVV、SVH检查。比较分析膜迷路积水患者在两种背景下SVV、SVH偏斜角度的临床特征。结果对照组30人均完成静态、动态SVV、SVH。静态背景下偏斜角度分别为(1.24±0.57)°、(1.31±0.93)°。动态背景下SVV、SVH偏斜角度为(4.56±3.11)°、(3.97±3.10)°。观察组44例均完成静态背景下SVV、SVH,36例完成动态SVV、SVH,8例感头晕不适未完成检查。静态SVV、SVH偏斜角度为(2.73±2.55)°、(2.94±2.70)°。观察组患者患侧旋转背景下SVV、SVH偏斜角度中位数为7.53(0.07-38.73)°、7.65(0.20-60.20)°。对侧旋转背景下SVV、SVH偏斜角度中位数为4.88(0.40-20.53)°、6.12(0.27-25.23)°。观察组静态背景下SVV、SVH偏斜角度大于对照组SVV、SVH偏斜角度,差异有统计学意义(P<0.05),SVV异常率40.9%,SVH异常率52.3%,两者差异无统计学意义(P>0.05)。SVV、SVH在动态背景下的偏斜角度比在静态背景下的偏斜角度大,差异有统计学意义(P<0.05)。动态背景下向患侧旋转时偏斜角度比向对侧旋转时偏斜角度大,差异有统计学意义(P<0.05)。结论正常人静态和动态背景下SVV、SVH偏斜角度对称。膜迷路积水患者静态SVV、SVH偏斜角度增大。在动态背景下,其表现为向患侧旋转背景时偏斜角度大于向对侧旋转背景下的偏斜角度。 Objective To analyze the characteristics of subjective visual vertical(SVV)and subjective visual horizontal(SVH)deviation angles of patients with endolymphatic hydrops under the static and dynamic backgrounds.Methods Forty-four patients with endolymphatic hydrops were recruited as study group and 30 healthy subjects as control group.SVV and SVH were performed on all subjects under static and dynamic backgrounds.The characteristics of SVV and SVH deviation angles under the two backgrounds were compared and analyzed,in patients with endolymphatic hydrops.Results All 30 healthy subjects completed the static and dynamic SVV and SVH.Under the static background,the deviation angles were(1.24±0.57)°and(1.31±0.93)°,respectively.Under the dynamic background,the deviation angles were(4.56±3.11)°and(3.97±3.10)°,respectively.In the study group,44 subjects completed SVV and SVH under static background,36 subjects completed dynamic SVV and SVH,and 8 subjects didn't complete the examination,because of feeling dizzy and uncomfortable.The static SVV and SVH deviation angles were(2.73±2.55)°and(2.94±2.70)°,respectively.As the background rotated towards the affected ear,the SVV and SVH deviation angles were 7.53°(0.07°-38.73°)and 7.65°(0.20°-60.20°),respectively.As the background rotated towards the contralateral ear,the median SVV and SVH deviation angles were 4.88°(0.40°-20.53°)and 6.12°(0.27°-25.23°),respectively.Under static background,the SVV and SVH skew angles of the study group were significantly greater than control group(P<0.05).The abnormal incidence of SVV and SVH were 40.9%and 52.3%,respectively,with no significant statistical difference.The deviation angle in dynamic background was significantly larger than in static background(P<0.05).Under dynamic background,the SVV and SVH deviation angles of rotation toward the affected ear were significantly larger than rotation toward the contralateral ear(P<0.05).Conclusion The deviation angles of SVV and SVH under the static and dynamic backgrounds are symmetric in control group.In patients with endolymphatic hydrops,the static SVV and SVH deviation angles increases.Under the dynamic background,the SVV and SVH deviation angles of rotation toward the affected ear are larger than rotation toward the contralateral ear.
