血清尿酸、尿酸/肌酐比值对AECOPD患者预后的评估价值

摘要 目的探讨血清尿酸(UA)、尿酸/肌酐(UA/Cr)比值水平对慢性阻塞性肺疾病急性加重期(AECOPD)患者预后的评估价值。方法选取2018年2月—2021年2月辽宁省某医院收治的150例AECOPD患者,根据患者预后设为死亡组(n=90)和存活组(n=60)。采用多因素Logistic回归分析影响患者预后的危险因素,采用ROC曲线分析血清UA及UA/Cr比值水平对预后的评估价值。结果不同预后AECOPD患者的血清UA、UA/Cr比值、动脉血氧分压、空腹血糖、C反应蛋白及白细胞计数比较,差异均有统计学意义(P<0.05)。Logistic回归分析结果显示,血清UA及UA/Cr比值水平为影响AECOPD患者预后的独立危险因素。ROC曲线分析结果显示,血清UA水平评估AECOPD患者预后的AUC为0.731(95%CI:0.651~0.810,P<0.05),UA/Cr比值评估AECOPD患者预后的AUC为0.744(95%CI:0.662~0.826,P<0.05)。结论血清UA及UA/Cr比值水平对评估AECOPD患者预后的价值较高,临床需加强这些指标的监测,以改善患者预后。 Objective To investigate the prognostic value of serum uric acid(UA) and uric acid/creatinine(UA/Cr) levels in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods One hundred and fifty patients with AECOPD admitted to a hospital in Liaoning Province from February 2018 to February 2021 were selected and set into a death group(n=90) and a survival group(n=60) according to the patients’ prognosis. Multi-factor logistic regression was used to analyze the risk factors affecting patients’ prognosis, and ROC curves were used to analyze the value of serum UA and UA/Cr levels in assessing prognosis. Results The levels of UA, UA/Cr, Pa O2, FBG, CRP and WBC in patients with AECOPD with different prognosis showed statistically significant differences(P<0.05). Logistic regression analysis showed that serum UA and UA/Cr levels were independent risk factors affecting the prognosis of patients with AECOPD. ROC curve analysis showed that the AUC of serum UA level to assess the prognosis of AECOPD patients was 0.731(95% CI:0.651-0.810, P<0.05), and the AUC of UA/Cr to assess the prognosis of AECOPD patients was 0.744(95% CI: 0.662-0.826, P<0.05). Conclusion Serum UA and UA/Cr levels are of high value in assessing the prognosis of patients with AECOPD, and clinical monitoring of these indicators needs to be strengthened to improve patient prognosis.
