基于人性化理念的全面引导护理在乳腺癌改良根治术患者中的应用

The application of comprehensive guidance care based on the concept of humanization in patients undergoing modified radical mastectomy for breast cancer

目的探讨基于人性化理念的全面引导护理在乳腺癌改良根治术患者中的应用价值。方法选取2019年1月—2020年12月在河南省某医院接受改良根治术治疗的81例女性乳腺癌患者,采用随机数字表法分为研究组(41例)与对照组(40例)。对照组患者给予常规护理,研究组患者给予基于人性化理念的全面引导护理。比较2组患者手术前后的汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)、日常生活能力评定量表(ADL)评分,术后并发症发生情况及护理满意度。结果 2组患者术后14 d HAMA及HAMD评分均低于术前1 d,且研究组低于对照组,差异均有统计学意义(P<0.05)。2组患者术后14 d ADL中进阶能力、基本能力维度评分及总分低于术前1 d,但研究组高于对照组,差异均有统计学意义(P<0.05)。研究组患者护理满意度为97.56%,高于对照组的82.50%,差异有统计学意义(χ^(2)=5.160,P=0.023)。研究组患者术后并发症发生率为9.76%,低于对照组的20.00%,但差异无统计学意义(χ^(2)=1.684,P=0.194)。结论基于人性化理念的全面引导护理可有效减轻乳腺癌根治术患者围术期负性情绪,改善患者生活质量。

Objective To explore the value of applying comprehensive guidance care based on the concept of humanization in patients undergoing modified radical mastectomy for breast cancer. Methods Eighty-one female breast cancer patients treated with modified radical surgery in a hospital in Henan Province from January 2019 to December 2020 were selected and divided into a study group(41 cases) and a control group(40 cases) using the random number table method. Patients in the control group were given conventional care and patients in the study group were given comprehensive guidance care based on the concept of humanization. The Hamilton Anxiety Rating Scale(HAMA), Hamilton Depression Rating Scale(HAMD), and Activities of Daily Living(ADL) scores, the occurrence of postoperative complications and nursing satisfaction were compared between the 2 groups before and after surgery. Results The HAMA and HAMD scores of patients in both groups were lower at 14 days after surgery than at 1 day before surgery, and the study group was lower than the control group, and the differences were statistically significant(P <0.05). the scores of advanced ability and basic ability dimensions and total scores in ADLs of patients in both groups were lower at 14 days after surgery than at 1 day before surgery, but the study group was higher than the control group, and the differences were statistically significant(P<0.05). Patient care satisfaction was 97.56% in the study group, which was higher than 82.50% in the control group, and the difference was statistically significant(χ^(2)=5.160, P=0.023). The postoperative complication rate of patients in the study group was 9.76%, which was lower than 20.00% in the control group, but the difference was not statistically significant(χ^(2)=1.684, P=0.194). Conclusion Comprehensive guidance care based on the concept of humanization can effectively reduce negative perioperative emotions and improve the quality of life of patients undergoing radical mastectomy for breast cancer.
