上海市某社区全科病房院内感染情况分析与讨论

Analysis and Discussion of Nosocomial Infection in General Ward of a Community in Shanghai

目的:分析社区卫生服务中心全科病房的院内感染信息,探讨其存在的问题及改进方法。方法:选取2018年1月-2020年12月上海市高境社区卫生服务中心全科病房收治的患者,描述性分析其年龄、院内感染类型、致病菌种类、住院时间及转归情况。结果:2018年1月-2020年12月高境社区卫生服务中心全科病房共计收住261人次,平均住院天数为(150.22±45.21)d,平均年龄为(81.93±11.33)岁;发生院内感染101人次,月院内感染发生率为6.3%;排名前三的院内感染部位为下呼吸道、上呼吸道、泌尿道;院感发生率随患者年龄增长而上升;与合并慢性疾病的数量比较,差异有统计学意义(P<0.05)。因院内感染死亡患者占死亡总数的47.83%。结论:高境社区全科病房院内感染发生率与年龄及合并慢性疾病的数量呈现正相关;呼吸系统为最易发生院内感染的部位,发生院内感染后死亡率较高,应重视院内感染的预防与治疗。

Objective:To analyze the nosocomial infection information in the general ward of a community health service center,and discuss its existing problems and improvement methods.Methods:Patients admitted to the general ward of Shanghai Gaojing Community Health Service Center from January 2018 to December 2020 were selected,and their age,type of nosocomial infection,type of pathogenic bacteria,hospital stay and outcome were descriptively analyzed.Results:From January 2018 to December 2020,the general ward of Gaojing Community Health Service Center has admitted and treated a total of 261 people-times,with an average hospital stay of(150.22±45.21)days and an average age of(81.93±11.33)years.There were 101 person-times with nosocomial infections,and the monthly nosocomial infection rate was 6.3%.The top three nosocomial infection sites were the lower respiratory tract,upper respiratory tract,and urinary tract.The incidence of nosocomial infection increased with the age of patients.Compared with the number of complicated chronic diseases,the difference was statistically significant(P<0.05).Patients who died due to nosocomial infection accounted for 47.83%of the total deaths.Conclusion:The incidence of nosocomial infections in the general wards in Gaojing Community is positively correlated with the age and the number of complicated chronic diseases.Respiratory system is the most susceptible site of nosocomial infection,and the mortality after nosocomial infection is high.It is essential to focus on the prevention and treatment of nosocomial infection.
