甘精胰岛素、格列美脲联合甲巯咪唑治疗2型糖尿病合并甲亢的临床效果观察

Clinical Effect Observation of Insulin Glargine and Glimepiride Combined with Methimazole in Treatment of Type 2 Diabetes Mellitus Complicated with Hyperthyroidism

目的:探究甘精胰岛素、格列美脲联合甲巯咪唑治疗2型糖尿病合并甲亢的临床效果。方法:选择2019年12月-2020年12月在我院就诊68例2型糖尿病合并甲亢患者进行研究,采用随机数表法将患者随机分为两组,每组各34例。其中,对照组采用甲巯咪唑联合甘精胰岛素治疗,观察组采用甘精胰岛素、格列美脲联合甲巯咪唑治疗,治疗观察8周。比较两组的治疗总有效率、治疗前后患者血糖和甲状腺激素水平变化情况及不良反应发生率。结果:观察组治疗总有效率显著高于对照组(P<0.05)。治疗前,两组的血糖(空腹血糖、餐后2 h血糖)、糖化血红蛋白、甲状腺激素水平比较差异无统计学意义(P>0.05)。治疗后,两组患者血糖、糖化血红蛋白、甲状腺激素水平均较治疗前有改善,且观察组显著优于对照组(P<0.05)。两组不良反应发生率对比差异无统计学意义(P>0.05)。结论:甘精胰岛素、格列美脲联合甲巯咪唑治疗2型糖尿病合并甲亢能够显著降低血糖水平,控制甲状腺激素水平,且安全性良好,值得推广应用。

Objective:To explore the clinical effect of insulin glargine,glimepiride combined with methimazole in the treatment of type 2 diabetes mellitus complicated with hyperthyroidism.Methods:Sixty-eight patients with type 2 diabetes mellitus complicated with hyperthyroidism admitted to our hospital from December 2019 to December 2020 were selected for the study.The patients were divided into two groups by random number table method,with 34 cases in each group.The control group was treated with methimazole combined with insulin glargine,and the observation group was treated with insulin glargine,glimepiride combined with methimazole,for 8 weeks.The total effective rate,changes in blood glucose and thyroid hormone levels before and after treatment and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of observation group was significantly higher than that of control group(P<0.05).Before treatment,there was no significant difference in blood glucose(fasting blood glucose,2 h postprandial blood glucose),glycosylated hemoglobin and thyroid hormone levels between the two groups(P>0.05).After treatment,blood glucose,glycosylated hemoglobin and thyroid hormone levels of patients in two groups were improved compared with before treatment,and observation group was significantly better than control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Insulin glargine,glimepiride combined with methimazole in the treatment of type 2 diabetes mellitus complicated with hyperthyroidism can significantly reduce blood glucose levels and control thyroid hormone levels,with good safety.It is worthy of promotion and application.
