三维斑点追踪技术评价肥厚型心肌病患者左室功能的价值

Evaluation of Left Ventricular Function in Patients with Hypertrophic Cardiomyopathy Using Three-Dimensional Speckle Tracking Technique

目的:评价三维斑点追踪技术评价肥厚型心肌病患者左室功能的临床价值。方法:选取2020年4月-2021年3月肥厚型心肌病患者50例,作为观察组;另选取同时间段内接受健康检查的健康人50例,作为对照组。两组均接受常规超声心动图检查及三维斑点追踪技术检查。比较两组检查结果及各项指标水平。结果:观察组室间隔舒张末期厚度、左心室后壁舒张末期厚度、E/EA、左心室流出道速度时间积分、左心室流出道最大压差均大于对照组,且左心室舒张末期前后径、E/A、Ea/Aa均小于对照组,差异有统计学意义(P<0.05)。三维径向应变指标中,两组前壁基底段、中段、心尖段以及后壁基底段水平比较,差异有统计学意义(P<0.05)。三维横向应变指标中,两组后间隔基底段和中段、前壁基底段和心尖段、前间隔基底段、侧壁基底段、后壁基底段水平比较,差异有统计学意义(P<0.05)。结论:采用三维斑点追踪技术可以评估肥厚型心肌病患者左心室的早期整体和局部心肌功能受损程度,更加精准,可全面测定心肌功能。

Objective:To evaluate the clinical value of three-dimensional speckle tracking technique in evaluating left ventricular function in patients with hypertrophic cardiomyopathy.Methods:Fifty patients with hypertrophic cardiomyopathy from April 2020 to March 2021 were selected as the observation group.Another 50 healthy people who underwent health examination in the same period were selected as the control group.Both groups received routine echocardiography and three-dimensional speckle tracking technique.The examination results and index levels of the two groups were compared.Results:In the observation group,the end-diastolic interventricular septum thickness,the end-diastolic thickness of left ventricular posterior wall,E/EA,the velocity time integral of left ventricular outflow tract and the maximum pressure difference of left ventricular outflow tract were higher than those in the control group,and the left ventricular end-diastolic anteroposterior diameter,E/A and Ea/Aa were lower than those in the control group,the difference was statistically significant(P<0.05).In the three-dimensional radial strain index,there was significant difference between the two groups in the levels of anterior wall basal segment,middle segment,apical segment and posterior wall basal segment(P<0.05).In the three-dimensional transverse strain indexes,there was significant difference between the two groups in the levels of posterior septal basal segment and middle segment,anterior wall basal segment and apical segment,anterior septal basal segment,lateral wall basal segment and posterior wall basal segment(P<0.05).Conclusion:Three-dimensional speckle tracking technology can evaluate the degree of early overall and local myocardial function damage of left ventricle in patients with hypertrophic cardiomyopathy,and it can more accurately and comprehensively measure myocardial function.
