急性缺血性脑卒中患者静脉溶栓治疗院内延误的调查与分析

Investigation and Analysis on In-Hospital Delay of Intravenous Thrombolytic Therapy in Patients with Acute Ischemic Stroke

目的:对急性缺血性脑卒中(AIS)患者静脉溶栓治疗流程进行调查,分析其主要救治时间点的现状。方法:将2020年1月-2021年7月收治的152例进行静脉溶栓治疗AIS患者作为研究对象,对比其"静脉溶栓质控表"与我国常规溶栓时间表,比较到院至给药时间(DNT)以及DNT时间达标与未达标患者各主要救治时间点情况。结果:初始CT扫描和CT判读的时间低于常规溶栓时间表,美国国立卫生研究院卒中量表(NIHSS)评分时间、卒中团队接诊时间,检验出结果时间及DNT时间均高于常规溶栓时间表,DNT时间达标患者主要救治时间点的时间小于DNT未达标患者。结论:AIS患者静脉溶栓治疗院内延迟严重,所以要不断优化流程,以缩短DNT时间,减少院内延迟。

Objective: To investigate the process of intravenous thrombolytic therapy in patients with acute ischemic stroke(AIS),and analyze the current situation of the main treatment time point. Methods: A total of 152 patients who underwent intravenous thrombolysis therapy from January 2020 to July 2021 were included as the objects, and the their "Quality Control Table of Thrombolysis" was compared with the schedule of routine thrombolysis in China, the door-to-needle time(DNT) and the main treatment time point between patients with compliant DNT time and patients with non-compliant DNT time were also compared.Results: The time of initial CT scan and CT reading were lower than that in the schedule of routine thrombolysis, the NIHSS score time, the stroke team visit time, the time of reporting test results and the time of DNT were higher than those in the schedule of routine thrombolysis. The time of main treatment time point in the patients with compliant DNT time was shorter than that in the patients with non-compliant DNT time. Conclusion: The in-hospital delay of intravenous thrombolytic therapy in AIS patients is serious, so it is necessary to continuously optimize the process so as to shorten DNT time and reduce in-hospital delay.
