地奥司明在全膝关节置换术后下肢水肿的治疗效果分析

Analysis on Treatment Effect of Diosmin for Lower Limb Edema After Total Knee Arthroplasty

目的:探究地奥司明对全膝关节置换术(TKA)术后下肢水肿的治疗效果。方法:选取2018年5月-2019年10月医院就诊的100例TKA术后下肢水肿患者,随机数字表法分为两组,各50例。观察组给予地奥司明治疗,对照组给予常规治疗,比较两组治疗后疼痛评分及炎性因子等指标。结果:观察组治疗7 d、21 d腓肠肌肿胀周径小于对照组、疼痛评分低于对照组,差异有统计学意义(P<0.05)。观察组治疗21 d白细胞介素-1、白细胞介素-6、肿瘤坏死因子-α水平均低于对照组,白细胞介素-10水平高于对照组,差异有统计学意义(P<0.05)。观察组治疗21 d骨钙素、骨型碱性磷酸酶、总Ⅰ型胶原氨基端延长肽水平均高于对照组,全段甲状旁腺素水平低于对照组,差异有统计学意义(P<0.05)。结论:地奥司明在TKA术后下肢水肿的社区治疗中,效果显著,且能够减轻其术后下肢水肿程度。

Objective:To explore the treatment effect of diosmin for lower limb edema after total knee arthroplasty(TKA).Methods:From May 2018 to October 2019,100 patients with lower limb edema after TKA were selected and were divided into two groups according to random number table method,with 50 cases in each group.The observation group was given diosmin treatment,and the control group was given routine treatment.After treatment,the pain score,inflammatory factor and other indicators in the two groups were compared.Results:The gastrocnemius muscle swelling circumference and pain score of the observation group were lower than those of the control group at 7 d and 21 d after treatment,with statistically significant difference(P<0.05).The levels of interleukin-1,interleukin-6 and tumor necrosis factor-αin the observation group were lower than those in the control group,while the levels of interleukin-10 were higher than those in the control group,with statistically significant difference(P<0.05).The levels of osteocalcin,bone alkaline phosphatase and total type I collagen amino-terminal elongation peptide in the observation group were higher than those in the control group at 21 d after treatment,while the levels of intact parathyroid hormone were lower than those in the control group,with statistically significant difference(P<0.05).Conclusion:Diosmin has significant effect in community treatment and follow-up of lower limb edema after TKA,and can reduce the degree of postoperative lower limb edema.
