肠系膜急性闭合性损伤的CT影像学特征及临床分析

CT imaging features and clinical analysis of acute closed injury of mesentery

目的通过对腹部闭合性外伤肠及肠系膜损伤的术前CT检查影像学表现与临床手术进行对比,探讨CT检查在腹部外伤肠及肠系膜损伤影像学表现及诊断优势,从而提高CT影像在肠及肠系膜损伤中的诊断符合率。方法选取2008至2020年67例中山市东升医院收治疑似腹部闭合性损伤患者,均实施CT检查,将患者的CT检查结果与确诊结果进行对比,总结CT影像检查的诊断符合率,不同肠系膜损伤患者的CT影像学特征。结果CT诊断符合率、漏诊率分别为98.5%、1.5%,与确诊结果比较,差异无统计学意义(P>0.05);CT检查的影像学特征有腹腔游离气体、腹腔以及肠间隙积血与积液、肠壁增厚水肿及肠壁血肿及肠系膜水肿。结论CT检查在肠系膜急性闭合性损伤患者诊断中效果理想,其影像学特征显著,可鉴别不同类型肠系膜损伤,且诊断符合率高。

Objective By com paringtheim agingfindings of preoperative CTexamination and clinical operation in intestinal and mesentery injuries caused by abdominal trauma,to explore the imaging findings and diagnostic advantages of CT examination in abdominal trauma intestinal and mesenteric injury,so as to improve the diagnostic coincidence rate of CT imaging in intestinal and mesenteric injury.Methods A total of 67 patients with suspected closed abdominal injury admitted to Dongsheng Hospital of Zhongshan city from 2008 to 2020 were selected,and a 1 of tliem underwent CT examination.The results of CT examination and pathological examination were compared to summarize the diagnostic coincidence rate of CT imaging,and the CT imaging characteristics of patients with different mesenteric injuries were summarized.Results The coincidence rate of CT diagnosis was 98.5%,and the rate of missed diagnosis was 1.5%.There was no significant differencc between CT examination and pathological exam ination(P>0.05).The imaging features of CT examination included free gas in abdominal cavity,blood and fluid accumulation in abdominal cavity and intestinal space,intestinal wall thickening and edema,intestinal wall hematoma and mesenteric Conclusion CT examination has good diagnostic value for patients with acute mesenteric closed injury.I s imaging characteristics are remarkable,and it can distinguish different types of mesenteric injury with high diagnostic coincidence rate.
