不同临床特征对卵巢子宫内膜异位囊肿剥除术后卵巢储备功能恢复的影响

The effect of different clinical characteristics on the recovery of ovarian reserve after removal of ovarian endometriotic cysts

目的分析不同临床特征对卵巢子宫内膜异位囊肿剥除术后卵巢储备功能恢复的影响。方法选取2010年8月至2020年7月台州市第一人民医院诊治的450例行卵巢子宫内膜异位囊肿剥除术的患者作为研究对象。记录不同临床特征患者术后12个月内的卵巢储备功能恢复情况,并采用多元线性回归分析影响术后卵巢储备功能恢复的因素。结果年龄､肿瘤大小､囊肿单双侧､止血方式､术中r-AFS分期可对术后12个月血清抗米勒管激素(AMH)恢复率构成影响(P<0.05);术前血清AMH与术后12个月血清AMH恢复率存在负相关(r=-0.280,P<0.05)。多元线性回归模型发现,年龄>35岁､双侧囊肿以及电凝止血可对术后12个月血清AMH恢复造成独立影响(P<0.05)。结论在患者行卵巢子宫内膜异位囊肿剥除术后,随着时间延长,血清AMH水平会有一定程度恢复,年龄>35岁､双侧囊肿以及电凝止血是影响术后卵巢储备功能恢复的因素,需引起重视。

Objective To analyze the effect of different clinical characteristics on the recovery of ovarian reserve after removal of ovarian endometriotic cysts.Methods 450 patients who underwent removal of ovarian endometriosis cysts in Taizhou First People′s Hospital were selected as the research object.The recovery of ovarian reserve in patients with different clinical characteristics within 12 months after surgery was recorded,and multiple linear regression was used to analyze the factors affecting the recovery of ovarian reserve after surgery.Results Age,tumor size,unilateral and bilateral cysts,hemostasis,intraoperative r-AFS staging can affect the recovery rate of serum anti-Müllerian hormone(AMH)at 12 months after surgery,and the differences between groups are statistically significant(P<0.05).Preoperative serum AMH was negatively correlated with the recovery rate of serum AMH at 12 months after operation(r=-0.280,P<0.05).The multiple linear regression model found that age>35 years old,bilateral cysts,and electrocoagulation hemostasis can independently affect the recovery of serum AMH at 12 months after surgery(P<0.05).Conclusions After removal of ovarian endometriotic cysts,the serum AMH level will recover to a certain extent with time.Age>35 years old,bilateral cysts and electrocoagulation are the factors affecting the recovery of postoperative ovarian reserve function,which should be paid attention to.
