苦参凝胶联合奥硝唑治疗耐药性滴虫性阴道炎的疗效及对炎性因子水平的影响研究

Effect of Kushen gel combined with ornidazole in the treatment of drug-resistant trichomonas vaginitis and its effect on inflammatory factors

目的探讨苦参凝胶联合奥硝唑治疗耐药性滴虫性阴道炎患者的疗效及对炎性因子水平的影响。方法选取2020年2月至12月舟山市普陀区人民医院收治的90例耐药性滴虫性阴道炎患者作为研究对象,根据随机数字表法将其分为对照组(n=45)和观察组(n=45)。对照组采用奥硝唑口服治疗,观察组采用苦参凝胶联合奥硝唑口服治疗。比较两组患者的临床疗效、临床症状消失时间、炎性因子水平、过氧化氢(H_(2)O_(2))阳性率、阴道乳酸菌检出率及药物相关的不良反应发生率。结果观察组的总有效率(93.33%)明显高于对照组(73.33%)(χ^(2)=16.824,P=0.001);观察组临床症状消失时间均较对照组明显缩短(P<0.05);治疗后观察组阴道灌洗液中白介素-8(IL-8)、白介素-13(IL-13)、白介素-2(IL-2)、肿瘤坏死因子-α(TNF-α)水平均明显低于对照组(t=6.571、P=0.001,t=5.437、P=0.001,t=4.961、P=0.011,t=6.275,P=0.001);观察组H_(2)O_(2)阳性率、阴道乳酸菌检出率均明显高于对照组(P<0.05);两组不良反应发生率比较,差异无统计学意义(χ^(2)=7.164,P=2.735)。结论苦参凝胶联合奥硝唑治疗耐药性滴虫性阴道炎患者疗效显著,可有效降低阴道分泌物炎性因子水平,改善阴道微生态环境,且安全可靠。

Objective To investigate the efficacy of Kushen gel combined with ornidazole in the treatment of drug-resistant trichomonas vaginitis and its influence on inflammatory factors.Methods 90 patients with drug-resistant trichomonas vaginitis treated in Zhoushan Putuo District People′s Hospital from February to December 2020 were selected as research objects,and divided into control group(n=45)and observation group(n=45)according to random number table method.The control group was treated with ornidazole orally,and the observation group was treated with Kushen gel combined with ornidazole orally.Clinical efficacy,clinical symptom disappear time,inflammatory factor level,positive rate of hydrogen peroxide(H_(2)O_(2)),vaginal lactobacillus detection rate and incidence of drug-related adverse reactions were compared between the two groups.Results The total effective rate of observation group(93.33%)was significantly higher than that of control group(73.33%)(χ^(2)=16.824,P=0.001).The time of clinical symptoms disappeared in observation group was significantly shorter than that in control group(P<0.05).After treatment,the levels of interleukin-8(IL-8),interleukin-13(IL-13),interleukin-2(IL-2)and tumor necrosis factor-α(TNF-α)in vaginal lavage fluid in the observation group were significantly lower than those in the control group(t=6.571,P=0.001,t=5.437,P=0.001,t=4.961,P=0.011,t=6.275,P=0.001).The positive rate of H_(2)O_(2) and vaginal lactobacillus in the observation group were significantly higher than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(χ^(2)=7.164,P=2.735).Conclusions Kushen gel combined with ornidazole is effective in the treatment of drug-resistant trichomonas vaginitis.It can effectively reduce the level of inflammatory factors in vaginal secretions,improve vaginal microenvironment,and is safe and reliable.
