阿法骨化醇在儿童过敏性紫癜中的临床应用研究

Clinical Analysis of alfa ossification alcohol in children with Henoch-Schonlein purpura

目的观察阿法骨化醇在儿童过敏性紫癜中的临床疗效。方法将宜宾市第一人民医院2015年~2019年收治的102例过敏性紫癜患儿按入院时间先后顺序随机分为治疗组53例、对照组49例。对照组按过敏性紫癜常规治疗,治疗组在常规治疗基础上加阿法骨化醇(0.25~0.5μg/d)口服。治疗后随访1年,观察两组患儿症状、体征消失时间,过敏性紫癜复发率,紫癜性肾脏损害(临床)的发生率及CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平的变化。结果治疗组各项临床症状的消失时间明显短于对照组,差异有统计学意义(P<0.05);治疗组患儿的过敏性紫癜复发率及紫癜性肾损害(临床)发生率均低于对照组,差异有统计学意义(P<0.05);治疗后3个月,治疗后治疗组的CD3^(+)、CD4^(+)水平和CD4^(+)/CD8^(+)比值明显高于对照组,差异有统计学意义(P<0.05)。结论使用阿法骨化醇治疗儿童过敏性紫癜能显著提高临床治疗效果及降低复发率。

Objective To explore the clinical effects of alfa ossification alcohol in children with Henoch-Schonlein purpura.Methods The 102 cases of Henoch-Schonlein purpura treated in Pediatrics of Yibin First People's Hospital from 2015 to 2019 were randomly divided into 2 groups according to the admission time:the treatment group(53 cases)and the control group(49 cases).The control group used conventional treatment,the treatment group received alfa ossification alcohol(0.25~0.5μg/d)on basis of the conventional treatment.The patients were follow upon 12 months.The symptom remission time、the incidence of the relapse of henoch-schonlein purpura、Incidence of purpura renal damage(clinical)and the level of CD3^(+)、CD4^(+)、CD8^(+) and CD4^(+)/CD8^(+) were recorded.Results The disappearance time of various clinical symptoms in the treatment group was significantly shorter than that in the control group,the difference was statistically significant(P<0.05);the recurrence rate of Henoch-Schonlein purpura and the incidence of Henoch-Schonlein purpura renal damage(clinical)in the treatment group were significantly lower than those in the control group(P<0.05);3 months after treatment,the CD3^(+)、CD4^(+) level and CD4^(+)/CD8^(+) in the treatment group were significantly higher than those in the control group(P<0.05).Conclusions The use of alfacalcidol in the treatment of Henoch-Schonlein purpura in children can significantly improve the clinical treatment effect and reduce the recurrence rate.
