儿童口腔颌面部间隙感染的回顾性分析

Retrospective analysis of oral and maxillofacial space infection in children

目的分析儿童口腔颌面部间隙感染的临床特点,为该病的防治提供依据。方法收集2010至2020年河北医科大学第二医院收治的107例口腔颌面部间隙感染患儿的病例资料进行回顾性分析,采用Pearson相关分析住院天数和发热天数与最高体温、入院时C-反应蛋白(CRP)值的相关性。结果107例口腔颌面部间隙感染患儿男女比例为1.28∶1.00;下颌下间隙感染最为常见,腺源性感染为主要感染源,金黄色葡萄球菌为易感菌;单纯抗炎治疗55例,自行破溃5例,脓肿切开引流47例;治愈率为96.26%;发热天数与最高体温、入院时CRP值呈正相关性(r值分别为0.054、0.206,P<0.05);住院天数与最高体温、入院时CRP值呈正相关(r值分别为0.473、0.226,P<0.05)。结论单纯抗炎治疗及手术治疗均为儿童颌面部间隙感染的有效治疗方法。住院天数和发热天数与最高体温、入院时CRP值具有相关性,对病情评估具有临床意义。

Objective To investigate the clinical features of oral and maxillofacial space infection in children,so as to provided basis for the prevention and treatment od the disease.Methods A total of 107 children with oral and maxillofacial infections who were treated in our hospital from January 2010 to December 2020 were enrolled in the study.Pearson correlation analysis was used to analyze the correlation among the length of hospital stay,the length of fever,the maximum temperature and C-reactive protein(CRP)levels.Results The male/female ratio of the patients was 1.28∶1.00.The submandibular space infection was the most common,in which,the main infection source was lymphadenitis infection.Staphylococcus aureus was the common pathogenic bacteria.Of the 107 patients,55 patients were treated by simple intravenous antibiotics,and there were 5 cases of spontaneous rupture,and 47 patients received surgical treatment,with healing rate being 96.26%.The length of fever was positively correlated with the maximum temperature,CRP levels(P<0.05),and the length of hospital stay was positively correlated with the CRP levels on admission(P<0.05).Conclusion Both simple intravenous antibiotics and surgical treatment are successful treatment ways.The length of fever and length of hospital stay are correlated with the maximum temperature and CRP levels,which has important clinical significanced for the evaluation of disease condition.
