休克指数在急诊创伤合并失血性休克急救中的作用

The significance of shock index in assessing the effects of emergency nursing care in emergency surgical trauma complicated by hemorrhagic shock

目的探讨分析休克指数在急诊创伤合并失血性休克急救护理效果中的作用。方法选取2019年9月至2020年8月收治的创伤合并失血性休克患者200例,采取红蓝球抽签方式对患者进行分组,蓝球患者接受常规护理100例为对照组,红球患者接受以休克指数为指导的急救护理干预100例为研究组,对2组患者最后的临床护理结局进行探讨和对比。结果研究组患者抢救成功率90.00%,与对照组(79.00%)比较较高(P<0.05);预后疗效评估中,与对照组患者比较,研究组患者获得的有效率明显较高(P<0.05);研究组患者出现并发症几率为5.00%,明显低于对照组(P<0.05);研究组患者凝血酶原时间、活化部分凝血活酶时间、血乳酸、混合静脉血氧饱和度、血红蛋白以及血小板水平值,与对照组患者比较改善结果较优(P<0.05)。2组患者各项临床指标干预前比较,差异无统计学意义(P>0.05)。结论以休克指数为基础的急救护理,对于创伤合并失血性休克患者而言,可以有效提高患者抢救成功率,降低并发症发生率,改善患者各项血气指标,最终提高患者预后效果。

Objective To investigate the significance of shock index in assessing the effects of emergency nursing care in emergency surgical trauma complicated by hemorrhagic shock.Methods Two hundred patients with emergency surgical trauma complicated by hemorrhagic shock who were treated in our hospital from September 2019 to August 2020 were enrolled in the study,who were divided into two groups according to red and blue ball lot,with 100 cases in each group.The patients in control group were treated by conventional nursing,however,the patients in research group were treated by emergency nursing intervention based on shock indexes.The final clinical nursing outcomes were observed and compared between the two groups.Results The rescue success rate in research group was significantly higher than that in control group(90.00%vs 79.00%,P<0.05).and the rescue success rate was significantly higher than that in control group(P<0.05).However the incidence rate of complications in research group was significantly lower than that in control group(P<0.05).Moreover,the shock indexes including prothrombin time,activated partial thromboplastin time,blood lactate,mixed venous oxygen saturation,hemoglobin,and platelet levels in research group were superior to those in control group(P<0.05).Before intervention there were no significant differences in the clinical indexes between the two groups(P>0.05).Conclusion The emergency nursing intervention based on shock indexes can effectively improve the shock indexes and the success rate of rescue for patients with surgical trauma complicated by hemorrhagic shock,which can reduce the incidence of complications,so as to improve the prognosis of patients.
