蚕食清创法联合封闭负压引流术治疗糖尿病足的临床研究

The clinical efficacy of nibbling debridement combined with closed negative pressure drainage in treating diabetic foot

目的研究蚕食清创法联合封闭负压引流术治疗糖尿病足的临床效果。方法选择2017年12月至2019年12月收治糖尿病足患者226例,采用随机数表法分为观察组(n=126)和对照组(n=100)。对照组采取常规清创治疗,观察组采用封闭负压引流技术联合蚕食清创处理治疗。评价2组治疗效果,并检测治疗前及7 d后转化生长因子-β(TGFβⅠ)及其Ⅱ型受体(TβRⅡ)阳性指数水平。结果观察组治疗后换药次数、抗生素使用时间、创面愈合时间及住院时间均优于对照组,差异有统计学意义(P<0.05)。治疗前,2组TGFβⅠ指数、TβRⅡ指数比较,差异无统计学意义(P<0.05);治疗7 d后,观察组TGFβⅠ指数、TβRⅡ指数均高于对照组,差异有统计学意义(P<0.05)。结论封闭负压引流技术联合蚕食清创处理治疗糖尿病足的临床效果显著,可加速创面愈合,促进患者恢复。

Objective To investigate the clinical efficacy of nibbling debridement combined with closed negative pressure drainage in treating diabetic foot.Methods A total of 226 patients with diabetic foot who were treated in our hospital from December 2017 to December 2019 were enrolled in thestudy,who were randomly divided into control group(n=100)and observation group(n=126).The patients in control group were treated with conventional debridement,however,thepatients in observation group were treated by closed negative pressure drainage combined with nibbling debridement.The curative effects,and the growth factor levels were observed and compared between the two groups.Results After treatment,the numbers of drug changes,the antibiotic use time,wound healing time,hospital stay and the number of dressing changes in observation group were superior to those in control group(P<0.05).Before treatment,,there were no significant differences in TGFβⅠindex and TβRⅡindex between the two groups(P>0.05).After 7-day treatment,TGFβⅠindex and TβRⅡindex in observation group were significantly higher than those in control group(P<0.05).Conclusion The closed negative pressure drainage technology combined with nibbling debridement has significant curative effects on diabetic foot,which can accelerate wound healing and promote the recovery of patients.
