肺楔形切除术治疗Ⅰ-b期老年NSCLC患者的临床效果及对生存率的影响

Clinical effects of pulmonary wedge resection onⅠ-b period NSCLC in elderly patients and its effects on the survival rate of patients

目的探讨肺楔形切除术治疗Ⅰ-b期老年非小细胞肺癌(NSCLC)患者的临床效果及对生存率的影响。方法将2017年1~12月接受治疗的120例Ⅰ-b期老年NSCLC患者纳入此次研究,行数字双盲法分为对照组与研究组,分别给予肺叶切除手术治疗和肺楔形切除术治疗,比较患者手术治疗情况、手术后并发症情况、生存状况。结果患者置管时间、淋巴清扫数、手术后疼痛情况评分差异无统计学意义(P>0.05),研究组手术中出血量低于对照组(P<0.05),手术进行时间、手术后住院时间短于对照组(P<0.05);研究组并发症发生率略低于对照组,但组间差异无统计学意义(P>0.05);患者1年到3年生存情况、3年局部复发情况、远处转移情况、死亡情况比较差异无统计学意义(P>0.05)。结论Ⅰ-b期老年NSCLC患者接受肺楔形切除术治疗,同肺叶切除手术比较,两种术式在并发症、远期生存情况方面效果无明显差异,预后情况尚可,但肺楔形切术能够保留更多肺组织,在缩短手术进行时间、住院治疗时间,减少手术中出血量,降低手术创伤方面效果更为理想,值得临床进行深入研究。

Objective To investigate the clinical effects of pulmonary wedge resection onⅠ-b period NSCLC in elderly patients and its effects on the survival rate of patients.Methods A total of 120 elderly patients withⅠ-b period NSCLC who were treated in our hospital from January 2017 to December 2017 were enrolled in the study.Accoring to digital double blind method,these patients were divided into control group and research group.The patients in control group were treated by lobectomy surgery,however,the patients in research group were treated by lung wedge resection.The operation status,and the complications after surgical treatment as well as survival status of patients were observed and compared between the two groups.Results There were no significant differences in the scores of catheterization time,number of lymphatic dissection and postoperative pain between the two groups(P>0.05).The amount of blood loss during operation in research group was significantly lower than that in control group(P<0.05),and the operation duration and postoperative hospital stay were significantly shorter than those in control group(P<0.05).There were no significant differences in the incidence of complications,and 1~3 year survival rate,3-year local recurrence rate,distant metastasis and death status between the two groups(P>0.05).Conclusion As compared with lobectomy surgery,the pulmonary wedge resection in treatment ofⅠ-b period NSCLC in elderly patients can keep more lung tissue,shorten operation time and hospitalization time,reduce intraoperative blood loss,reduce the surgical trauma,therefore,it is worth further clinical research.
