持续静脉微量泵入氢化可的松联合胸腺肽α1对脓毒症休克患者血清炎性因子水平的影响

Effect of Continuous Intravenous Micropumping of Hydrocortisone Combined with Thymosin α;on Serum Inflammatory Factors in Patients with Septic Shock

摘要 目的:探讨持续静脉微量泵入氢化可的松联合胸腺肽α_(1)对脓毒症休克患者血清炎性因子水平的影响。方法:将莆田市第一医院2019年10月-2020年10月收治的80例脓毒症休克患者按入院顺序分为对照组(40例)和观察组(40例)。对照组给予持续静脉微量泵入氢化可的松治疗,观察组在对照组的基础上增加胸腺肽α_(1)。对比两组临床疗效、临床指标及血清炎性因子水平。结果:观察组治疗总有效率高于对照组(P<0.05)。观察组休克持续时间、总住院时间均短于对照组(P<0.05)。治疗7 d后,两组的血清降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)水平均下降,且观察组低于对照组(P<0.05);治疗7 d后,两组的血清CD3^(+)、CD4^(+)水平均升高,且观察组高于对照组,两组的血清CD8^(+)均下降,且观察组低于对照组(P<0.05)。结论:持续静脉微量泵入氢化可的松联合胸腺肽α_(1)可降低脓毒症休克患者的炎症水平,并改善患者的免疫功能,可有效促进患者恢复,缩短休克时间,提高临床治疗效果。 Objective:To explore the effect of continuous intravenous micropumping of Hydrocortisone combined with Thymosin α_(1) on serum inflammatory factors in patients with septic shock.Method:A total of 80 patients with septic shock in Putian First Hospital from October 2019 to October 2020 were divided into the control group(40 cases)and the observation group(40 cases)according to the order of admission.The control group was given continuous intravenous micropumping of Hydrocortisone,and the observation group was given Thymosin α_(1) on the basis of the control group.The clinical efficacy,clinical indicators and serum inflammatory factors were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group(P<0.05).The duration of shock and total hospital stay in the observation group were shorter than those in the control group(P<0.05).After 7 days of treatment,the levels of serum procalcitonin(PCT),tumor necrosis factor-α(TNF-α)and high-sensitivity C-reactive protein(hs-CRP)in both groups decreased,and which in the observation group were lower than those in the control group(P<0.05).After 7 days of treatment,the levels of serum CD3^(+) and CD4^(+) in both groups increased,and the observation group was higher than the control group;the serum CD8^(+) levels in both groups decreased,and which in the observation group were lower than those in the control group(P<0.05).Conclusion:Continuous intravenous micropumping of hydrocortisone combined with Thymosin α_(1) can reduce the level of inflammation in patients with septic shock and improve the immune function of patients,which can effectively promote the recovery of patients,shorten the time of shock,and improve the clinical treatment effect.
