脑脊液置换疗法治疗蛛网膜下腔出血并发脑血管痉挛的效果

Effect of Cerebrospinal Fluid Replacement Therapy in the Treatment of Subarachnoid Hemorrhage Complicated with Cerebral Vasospasm

目的:探讨脑脊液置换疗法治疗蛛网膜下腔出血并发脑血管痉挛的效果。方法:选取2019年1月-2021年1月惠东县人民医院收治的80例蛛网膜下腔出血并发脑血管痉挛患者,按照入院顺序将其分为观察组与对照组,每组40例。对照组采用尼莫地平治疗,观察组患者在此基础上接受脑脊液置换疗法干预。比较两组神经功能缺损评分(MESSS评分)、Glasgow预后评分、疗效及不良反应发生情况。结果:观察组总有效率为85.00%,高于对照组的52.50%,差异有统计学意义(P<0.05)。治疗后,观察组MESSS评分低于对照组,Glasgow预后评分高于对照组,差异均有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:蛛网膜下腔出血并发脑血管痉挛患者接受脑脊液置换疗法干预,能够有效改善临床症状,降低神经功能缺损,提升患者预后。此外,脑脊液置换疗法并不会增加不良反应发生风险,具有较高的安全性,值得临床推广。

Objective:To investigate the effect of cerebrospinal fluid replacement therapy in the treatment of subarachnoid hemorrhage complicated with cerebral vasospasm.Method:A total of 80 patients with subarachnoid hemorrhage complicated with cerebral vasospasm admitted to Huidong County People’s Hospital from January 2019 to January 2021 were selected,they were divided into the observation group and the control group according to the order of admission,40 cases in each group.The control group was treated with Nimodipine,and the observation group received cerebrospinal fluid replacement therapy on this basis.The neurological deficit score (MESSS score),Glasgow prognosis score,comprehensive efficacy and the occurrence of adverse reactions were compared between two groups.Result:The total effective rate of observation group was 85.00%,which was higher than 52.50% of the control group,the difference was statistically significant (P<0.05).After treatment,MESSS score of the observation group was lower than that of the control group,Glasgow prognosis score of the observation group was higher than that of the control group,the differences were statistically significant (P<0.05).There was no significant difference in the incidence of adverse reactions between two groups (P>0.05).Conclusion:The intervention of cerebrospinal fluid replacement therapy can effectively improve clinical symptoms,reduce neurological function defects and improve the prognosis of subarachnoid hemorrhage complicated with cerebral vasospasm patients.In addition,cerebrospinal fluid replacement therapy does not increase the risk of adverse reactions,and it is safe and worthy of clinical promotion.
