中药热敷联合穴位按摩在气滞血瘀证盆腔炎性疾病后遗症中的应用效果

Application effect of traditional Chinese medicine hot compress combined with acupoint massage in the treatment of sequelae of pelvic inflammatory disease with syndrome of qi stagnation and blood stasis

目的探讨中药热敷联合穴位按摩在气滞血瘀证盆腔炎性疾病(PID)后遗症中的应用效果。方法选择2018年5月至2020年10月江苏省南通市第一人民医院收治的气滞血瘀证PID后遗症患者80例,采用随机数字表法将其分为观察组(40例)和对照组(40例)。对照组给予西药联合穴位按摩治疗,观察组在对照组基础上给予中药热敷。治疗3个疗程后,比较两组临床疗效、中医证候评分、体征积分、总积分、血清炎性因子及血液流变学的变化。结果随访过程中观察组失访2例,对照组失访4例,最终共纳入患者74例,观察组38例,对照组36例。观察组临床疗效优于对照组,差异有统计学意义(P<0.05)。治疗后两组中医证候评分、体征积分、总积分低于治疗前,且观察组低于对照组(P<0.05)。治疗后两组血清白细胞介素-8、肿瘤坏死因子-α低于治疗前,且观察组低于对照组;两组血清IL-10高于治疗前,且观察组高于对照组(P<0.05)。治疗后两组高切全血黏度、低切全血黏度、血浆黏度、血细胞比容均低于治疗前,且观察组低于对照组(P<0.05)。结论中药热敷联合穴位按摩有助于改善PID后遗症患者临床症状,提高治疗效果,这可能与抑制炎症反应、调节血液流变学有关。

Objective To explore the application effect of traditional Chinese medicine hot compress combined with acupoint massage in the treatment of sequelae of pelvic inflammatory disease(PID)with syndrome of qi stagnation and blood stasis.Methods A total of 80 PID sequelae patients with syndrome of qi stagnation and blood stasis admitted to First People’s Hospital of Nantong City from May 2018 to October 2020 were selected,and they were divided into the observation group(40 cases)and the control group(40 cases)by random number table method.The control group was given Western medicine combined with acupoint massage therapy,and the observation group was given traditional Chinese medicine hot compress on the basis of the control group.After three courses of treatment,clinical efficacy,the changes of traditional Chinese medicine syndromes scores,signs integral,total integral,serum inflammatory factors,and hemorheology were compared between the two groups.Results During the follow-up,two patients in the observation group and fours patients in the control group were lost to follow-up.A total of 74 patients were included,38 in the observation group and 36 in the control group.The effective rate of the observation group was better than that of the control group,the difference was statistically significant(P<0.05).The traditional Chinese medicine syndromes scores,signs integral,and total integral of the two groups after treatment were lower than those before treatment(P<0.05),and those of the observation group were lower than the control group(P<0.05).The serum interleukin-8(IL-8)and tumor necrosis factor-αof the two groups after treatment were lower than those before treatment,and the observation group were lower than the control group(P<0.05);the IL-10 levels of the two groups after treatment were higher than those before treatment,and those of the observation group were higher than the control group(P<0.05).The high-cut whole blood viscosity,low-cut whole blood viscosity,plasma viscosity,and hematocrit of the two groups after treatment were lower than those before treatment,and thoseof the observation group were lower than control group(P<0.05).Conclusion The combination of traditional Chinese medicine hot compress and acupoint massage can improve the clinical symptoms of PID sequelae patients,improve treatment effect,which may be related to inhibit inflammation and regulate blood rheology.
