基于数据挖掘分析中医药治疗卒中后便秘的用药规律

An analysis of the regularity of treating constipation after stroke in TCM based on data mining

目的:利用古今医案云平台(V2.3.8)软件,分析把握中医药治疗卒中后便秘的用药规律。方法:收集建库至2021年3月10日被中国知网、维普、万方数据知识服务平台、中国生物医学文献数据库等平台收录的中医药治疗卒中后便秘的文献,收集中药处方,建立符合要求的处方数据库,建立Excel数据表,并将相关数据录入古今医案云平台(V2.3.8)。利用软件的数据挖掘功能进行频次统计、中药属性分析、关联规则分析、复杂网络分析、聚类分析,探索卒中后便秘的用药规律及核心处方。结果:去除不符合标准文献后,共纳入51篇文献,录入中药处方51首,合计95味中药,总用药频次386次。频次高的有大黄、枳实、当归、厚朴;药性以寒、温、微寒为先,五味则多苦、甘、辛,用药大多归脾经、胃经、肺经、肝经、大肠经;核心药对有厚朴-枳实、大黄-芒硝、麦冬-地黄等;核心处方以大承气汤、麻子仁丸、增液行气汤为基础加减。结论:中医药治疗卒中后便秘用药以润肠通便、凉血解毒等为重要治法,为临床治疗提供一定参考。

Objective:To analyze the regularity of treating constipation after stroke in TCM based on data mining by ancient and modern medical records cloud platform software(V2.3.8).Methods:From the establishment of the database to March 10,2021,the literature of treating post-stroke constipation in TCM included in CNKI,VIP,Wanfang Data and CBM were collected.Chinese medicine prescriptions were collected,the prescription database that meets the requirements was established,and the Excel data table was established,relevant data was entered into the ancient and modern medical records cloud platform(V2.3.8).Frequency statistics,TCM attribute analysis,association rule analysis,complex network analysis,cluster analysis were carried out by the data mining function of the software,the medication rules and core prescriptions for post-stroke constipation have been explored.Results:After removing the documents that did not meet the standards,a total of 51 documents were included,51 Chinese medicine prescriptions were entered,a total of 95 Chinese medicines,and the total frequency of medication was 386 times.The most frequent ones are Dahuang(Radix et Rhizoma Rhei),Zhishi(Fructus Aurantii Immaturus),Danggui(Radix Angelicae Sinensis),Houpo(Cortex Magnoliae Officinalis).The medicinal properties are more cold,warm and slightly cold.There are more bitter,sweet and pungent in five flavours.Most of the medications belong to spleen meridian,stomach meridian,lung meridian,liver meridian,large intestine meridian.Core drug pairs include Houpo-Zhishi,Dahuang-Mangxiao(Natrii Sulfas),Maidong(Radix Ophiopogonis)-Dihuang(Radix Rehmanniae),etc..The core prescriptions are the Dachengqi decoction(大承气汤),Maziren Wan(麻子仁丸)and the Zengye Xingqi decoction(增液行气汤).Conclusion:Most Chinese medicines used to treat post-stroke constipation have the effects of loosen the bowels to relieve constipation,cool the blood and remove the toxic materials.
