分析腔镜辅助颈部小切口甲状腺手术与传统开放式甲状腺手术治疗甲状腺良性肿瘤的效果及安全性

Analysis of the effect and safety of endoscope-assisted small incision thyroid surgery and traditional open thyroid surgery in patients with benign thyroid tumor

目的研究甲状腺良性肿瘤患者应用腔镜辅助颈部小切口甲状腺手术与传统开放式甲状腺手术治疗的效果差异及安全性。方法100例甲状腺良性肿瘤患者,按照随机单盲法分为对照组及观察组,每组50例。对照组采用传统开放式甲状腺手术治疗,观察组采用腔镜辅助颈部小切口甲状腺手术治疗。比较两组并发症发生情况、术中出血量、术后引流量、切口大小、住院时间及复发情况。结果观察组并发症发生率8.00%低于对照组的24.00%,差异具有统计学意义(P<0.05)。观察组术中出血量(13.6±3.6)ml、术后引流量(26.8±6.0)ml少于对照组的(31.2±4.5)、(37.8±6.5)ml,住院时间(3.2±0.4)d、切口大小(2.3±0.5)cm短于对照组的(4.0±0.6)d、(5.8±0.6)cm,差异具有统计学意义(P<0.05)。观察组复发率低于对照组,但差异无统计学意义(P>0.05)。结论腔镜辅助小切口甲状腺手术在治疗甲状腺良性肿瘤中具备的优势更明显,对患者的伤害比较小,安全度高,有利于患者术后恢复。

Objective To study the effect and safety of endoscope-assisted small incision thyroid surgery and traditional open thyroid surgery in patients with benign thyroid tumor.Methods A total of 100 patients with benign thyroid tumor were randomly divided into control group and observation group according to random singleblind method,with 50 cases in each group.The control group was treated with traditional open thyroid surgery,and the observation group was treated with endoscopic-assisted small incision thyroid surgery.The complications,intraoperative blood loss,postoperative drainage,incision size,hospitalization time and recurrence were compared between the two groups.Results The complication rate 8.00% in the observation group was lower than 24.00% in the control group,and the difference was statistically significant(P<0.05).The intraoperative blood loss(13.6±3.6)ml and postoperative drainage volume(26.8±6.0)ml of the observation group were less than(31.2±4.5)and(37.8±6.5)ml of the control group;and the hospitalization time(3.2±0.4)d and incision size(2.3±0.5)cm of the observation group were shorter than(4.0±0.6)d and(5.8±0.6)cm of the control group;all the differences were statistically significant(P<0.05).The recurrence rate of the observation group was lower than that of the control group,but the difference was not statistically significant(P>0.05).Conclusion In the treatment of benign thyroid tumor,endoscopic assisted small incision surgery has more obvious advantages of less injury to patients,high safety,which is beneficial to patient's postoperative recovery.
