96例早期周围型肺癌的CT影像特征分析

Analysis of CT imaging features of 96 cases of early-stage peripheral lung cancer

目的探究早期周围型肺癌的CT影像特征。方法96例早期周围型肺癌患者作为研究对象,依照患者病灶直径大小分为A组(病灶直径1~2 cm,52个病灶)和B组(病灶直径<1 cm,51个病灶),每组48例。所有患者均经CT检查做出影像学诊断,比较两组患者病灶的CT影像特征。结果两组患者的病灶密度比较,差异无统计学意义(P>0.05)。B组患者的瘤体边缘光滑征占比55.77%高于A组的21.57%,分叶征占比26.92%、毛刺征占比17.31%低于A组的76.47%、66.67%,差异具有统计学意义(P<0.05)。两组患者的支气管征+空泡+假空洞征、异常血管集束+穿行征、胸膜凹陷征占比比较,差异无统计学意义(P>0.05)。结论在周围型肺癌的CT影像中,分叶征、毛刺征等会随肺癌进展而发生变化,但早期占比并不高,且病灶密度无特异性,临床诊断中易造成误诊、漏诊状况,而观测支气管征、空泡征、假性空洞等征象可有效鉴别早期周围型肺癌。

Objective To investigate the CT imaging features of early-stage peripheral lung cancer.Methods A total of 96 patients with early-stage peripheral lung cancer were studied and divided into group A(52 lesions with a lesion diameter of 1-2 cm)and group B(51 lesions with a lesion diameter of<1 cm)according to the size of the lesion diameter,with 48 patients in each group.All patients were diagnosed by CT,and the CT imaging features of the lesions in the two groups were compared.Results There was no statistically significant difference in the density of lesions between the two groups(P>0.05).Group B had higher proportion of smooth edge sign around tumor of 55.77% than 21.57% in group A,and lower proportion of lobulation sign and burr sign of 26.92% and 17.31% than 76.47% and 66.67% in group A,and the differences were statistically significant(P<0.05).The differences were not statistically significant when comparing the proportion of bronchial sign+vacuole+false cavity sign,abnormal vascular cluster+perforating sign and pleural indentation sign in the two groups(P>0.05).Conclusion In the CT images of peripheral lung cancer,lobulation sign and burr sign will change with the progress of lung cancer,but the early-stage proportion is not high,and the lesion density is not specific,which is easy to cause misdiagnosis and missed diagnosis,and observation of bronchial sign,vacuole sign,false cavity and other signs can effectively identify early-stage peripheral lung cancer.
