胸腰椎术后穴位疗法对胃肠功能影响的Meta分析

A Meta-analysis of the effect of acupoint therapy on gastrointestinal function after thoracolumbar spine surgery

目的:应用累积Meta分析评价穴位疗法对胸腰椎术后患者胃肠功能的影响。方法:计算机检索中国生物医学文献数据库、中国期刊网全文数据库、万方数据库,及PubMed、Cochrane Library、Web of Science、Embase数据库,搜集关于胸腰椎术后患者穴位疗法的随机对照试验,检索时限为建库至2020年1月1日。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,按发表时间顺序进行累积Meta分析,同时采用试验序贯分析方法,评价纳入研究的样本量是否达到界值。结果:共纳入32项研究,包括3000例患者。累积Meta分析结果显示,穴位疗法可以促进胸腰椎术后患者早期排气、排便,缩短首次闻及肠鸣音时间,减少患者术后便秘、腹胀和恶心呕吐的发生率。同时,试验序贯分析方法Meta分析的样本量已达到界值,建议不需要进一步的研究。结论:现有证据显示,对胸腰椎术后患者实施穴位疗法能够有效调节POGD,促进胃肠功能恢复。

Objective:To evaluate the effect of acupoint therapy on the gastrointestinal function of patients after thoracolumbar spine surgery by cumulative Meta-analysis.Methods:The Chinese Biomedical Literature Database,CNKI,Wanfang Database,PubMed,Cochrane Library,Web of Science and Embase databases were searched by computer to collect randomized controlled trials on acupoint therapy for patients after thoracolumbar spine surgery.The retrieval period is from the database construction to January 1,2020.Two researchers independently screened the literature,extracted the data,and evaluated the bias risk of the included studies.Then,a cumulative meta-analysis was conducted according to the chronological order of publication,and sequential experimental analysis was used to evaluate whether the sample size of the included studies reached the threshold.Results:A total of 32 studies were included,including 3000 patients.Cumulative meta-analysis results showed that acupoint therapy can promote early exhaust and defecation,shorten the time of first bowel sound,and reduce the incidence of postoperative constipation,abdominal distention,nausea and vomiting.At the same time,the sample size of the sequential meta-analysis method had reached the threshold,suggesting that further research is not needed.Conclusion:Existing evidence shows that acupoint therapy can effectively regulate postoperative gastrointestinal dysfunction and promote recovery of gastrointestinal function for patients after thoracolumbar spine surgery.
