自拟益肾汤对糖尿病肾病的疗效和安全性考察

Study on the Efficacy and Safety of Self-made Yishen Decoction on Diabetic Nephropathy

目的探究自拟益肾汤对糖尿病肾病的改善程度和用药安全性。方法选取2019年1月—2020年12月该院收治的100例糖尿病肾病患者为研究对象。按照随机数表法分组,平分成对比组(常规西药治疗)、自拟益肾汤组(西药联合自拟益肾汤治疗)。对比血糖情况、肾功能情况、中医症候积分、疗效、不良反应。结果在血糖上,对比组FBG(8.30±0.61)mmol/L>自拟益肾汤组(7.37±0.68)mmol/L,对比组PBG(12.20±1.25)mmol/L>自拟益肾汤组(10.36±1.97)mmol/L,对比组HbA1c(7.69±4.24)%>自拟益肾汤组(6.26±2.30)%;在肾功能指标上,对比组UAER(0.12±0.02)g/24 h>自拟益肾汤组(0.08±0.01)g/24 h,对比组Scr(95.52±6.53)μmol/L>自拟益肾汤组(83.57±5.50)μmol/L,对比组BUN(6.54±0.72)mmol/L>自拟益肾汤组(5.77±0.65)mmol/L,差异有统计学意义(P<0.05);在中医症候积分上,对比组神倦气乏(3.50±1.05)分>自拟益肾汤组(2.20±0.82)分,对比组五心烦热(3.11±1.25)分>自拟益肾汤组(1.95±1.05)分,对比组口干咽燥(3.25±0.58)分>自拟益肾汤组(2.69±0.34)分,对比组肢体浮肿(2.36±1.14)分>自拟益肾汤组(1.34±0.60)分,差异有统计学意义(t=6.900、5.025、5.890、5.599,P<0.05);在疗效上,对比组有效率72.00%<自拟益肾汤组90.00%,差异有统计学意义(χ^(2)=5.263,P<0.05);在不良反应上,对比组不良反应发生率28.00%>自拟益肾汤组8.00%,差异有统计学意义(χ^(2)=6.775,P<0.05)。结论常规西药联合应用自拟益肾汤,可以显著降低血糖,改善肾功能,并且中医症候也得到较为明显改善,疗效可达90.00%,可以良好地控制糖尿病肾病进展。

Objective To explore the improvement of self-made Yishen Decoction on diabetic nephropathy and the safety of medication.Methods From January 2019 to December 2020,100 diabetic nephropathy patients in the hospital were selected as the subjects.They were divided into the comparison group(conventional Western medicine treatment)and Self-made Yishen Decoction group(Western medicine combined with self-made Yishen Decoction according to random number table method.Compare blood glucose,kidney function,TCM syndrome score,curative effect,and adverse reactions.Results In terms of blood glucose,FBG in the control group(8.30±0.61)mmol/L>self-made Yishen Decoction group(7.37±0.68)mmol/L,and PBG in the comparison group(12.20±1.25)mmol/L>self-made Yishen Decoction group(10.36±1.97)mmol/L,the comparison group HbA1c(7.69±4.24)%>self-made Yishen Decoction group(6.26±2.30)%;in terms of renal function indicators,the comparison group UAER(0.12±0.02)g/24 h>self-made Yishen Decoction group(0.08±0.01)g/24 h,the comparison group Scr(95.52±6.53)μmol/L>self-made Yishen Decoction group(83.57±5.50)μmol/L,the comparison group BUN(6.54±0.72)mmol/L>self-made Yishentang group(5.77±0.65)mmol/L,and the difference was statistically significant(P<0.05);in terms of TCM syndrome scores,the comparison group had mental fatigue and Qi deficiency(3.50±1.05)points>the self-made Yishen Decoction group(2.20±0.82)points,and the comparison group had five upset fevers(3.11±1.25))points>self-made Yishen Decoction group(1.95±1.05)points,dry mouth and dry throat in the Decoction group(3.25±0.58)>self-made Yishen Decoction group(2.69±0.34)points,edema of limbs in the Decoction group(2.36±1.14)points>self-made Yishen Decoction group(1.34±0.60)points,and the difference was statistically significant(t=6.900,5.025,5.890,5.599,P<0.05);in terms of curative effect,effective rate 72.00%in the comparison group<90.00%in the self-made Yishen Decoction group,and the difference was statistically significant(χ^(2)=5.263,P<0.05);in terms of adverse reactions,the incidence of adverse reaction in the comparison group 28.00%>the self-made Yishen Decoction group 8.00%,and the difference was statistically significant(χ^(2)=6.775,P<0.05).Conclusion Combined application of self-made Yishen Decoction with conventional western medicine can significantly reduce blood glucose and improve renal function.Among them,the medical symptoms have also been significantly improved,and the curative effect can reach 90.00%,which can well control the progression of diabetic nephropathy.
